Randomized Controlled Trial Comparative StudyComparison of the effects of letrozole and clomiphene citrate for ovulation induction in infertile women with polycystic ovary syndrome.
Ovulation dysfunction is one of the most common causes of reproductive failure in infertile couples. The prevalence of this disorder in infertile women is about 30 to 40%. Polycystic ovary syndrome is a common disease that is closely related to ovulation dysfunction and 7% of women of childbearing age are afflicted with it. Ovulation induction is a way to treat infertility in PCOS which is possible through medication or surgery. This study was conducted to compare the effects of Letrozole and Clomiphene citrate for ovulation induction in women with PCOS. ⋯ This study showed that stimulation of letrozole and clomiphene citrate on ovulation and was almost the same and clomiphene citrate caused endometrial thinning more often than letrozole. Also the side effects reported by patients in the group receiving clomiphene citrate were higher, while in the group receiving letrozole no complication was reported. Based on these findings letrozole can be considered an appropriate alternative for clomiphene citrate without side effects.

The use of diagnostic ultrasound and the diffusion of the technique improved the obstetric treatment and the usefulness of ultrasound increases in the delivery room for maternal and fetal care and as method of diagnosis of some obstetric complications. The knowledge of intrapartum ultrasound imaging can be considered useful for the obstetric team, since there is evidence that ultrasound can improve the obstetric management. ⋯ Besides, the use of ultrasound is reported in obstetric and postpartum complications. Actually ultrasonography, as a non-invasive, safety and low-cost technique, offers a diagnostic method in particular conditions during labour, delivery and postpartum.

ReviewThe role of neoadjuvant chemotherapy in the management of primary breast cancer.
Neoadjuvant chemotherapy for primary breast cancer is the gold standard in the treatment of locally advanced, inoperable breast cancer, but based on a large body of evidence has become a standard treatment option for patients with operable disease, who are clear candidates for adjuvant chemotherapy. Neoadjuvant chemotherapy aims at reducing mortality and improving surgical options and offers an in vivo chemosensitivity testing at the same time. ⋯ A reliable high quality diagnostic and pathological work-up is mandatory for an ideal tailoring of neoadjuvant chemotherapy. This review gives an outline of the state-of-the-art management of primary breast cancer in the setting of neoadjuvant chemotherapy.

Comparative StudyGestational diabetes and fetal growth acceleration: induction of labour versus expectant management.
The aim of the study was to compare elective induction of labour at 38 weeks versus expectant management in A1 and A2 gestational diabetes (GDM) pregnancies with fetal growth acceleration. Primary outcome of the study was C-section (CS) rate, while secondary outcomes were macrosomia incidence and adverse perinatal outcomes. ⋯ Labour induction at 38 weeks in GDM patients with fetal growth acceleration does not seem to determine an increased incidence of C-section in comparison to expectant management, particularly in case of maternal obesity.

Comparative StudyImpact of cafedrine/theodrenaline (Akrinor® ) on therapy of maternal hypotension during spinal anesthesia for Cesarean delivery: a retrospective study.
Maternal hypotension is the most frequent complication in spinal anesthesia for Cesarean delivery. Malperfusion of the foetus and nausea and vomiting of the mother are hallmarks of maternal hypotension. In this retrospective data analysis and anesthesia protocols we have investigated to explore the effects of therapeutic interventions for hypotension with cafedrine/theodrenaline (Akrinor® ) during spinal anesthesia for elective Cesarean section. ⋯ The results of this study confirm a rapid and sustained increase in blood pressure after application of Akrinor® for treatment of sympathicolysis induced hypotension. No negative impact of Akrinor® on umbilical arterial cord pH and APGAR scores was observed.
