Early recognition of shock is the key to successful resuscitation in critically ill children. Often, shock results in or co-exists with myo-cardial dysfunction or acute lung injury. Recognition and appropriate management of these insults is crucial for successful outcomes. ⋯ Management of co-morbidities such as asthma and seizures should be implemented simultaneously. Inotropes, respiratory support, antibiotics and steroids may also be needed during the management of shock. While the management of shock can be protocol based, the treatment needs to be individualized depending on the suspected etiology and therapeutic response.

The aim of this paper was to to provide the primary care practitioner with a review of genitourinary conditions in children requiring emergent urological referral. These conditions include the acute scrotum/testicular torsion, scrotal masses, penile abnormalities, urinary retention and bladder outlet obstruction, abdominal masses, acute urinary stones, urinary trauma, genital abnormalities, complex anomalies and acute abdomen in patients with previous continent reconstruction. The authors discussed the presenting signs and symptoms, proper initial diagnostic work-up and the usual therapeutic course of management after evaluation by the pediatric urologist.

The natural history of allergic disease and its potential for prevention merit close examination because of the explosive worldwide increase in the prevalence and morbidity of atopic disorders. In infants from ''high-risk'' families (i.e. those with one or two parents and/or a sibling with food allergy, eczema, asthma or allergic rhinitis) food allergen avoidance has been advocated as means of preventing the development of atopic disease. ⋯ When breast-feeding is not possible or supplemental feeding is needed, infants from atopic families should be given a hydrolyzed infant formula for the first 6 month of life. High-risk infants without a history of eczema in a primary relative will receive the protective effect from the less expensive partial hydrolyzed formula (p-HF); whereas those infants who have first-degree relatives with eczema should receive the extensively hydrolyzed formula (e-HF).

[Naples: the historic capital of Italian paediatrics].
No other Italian city has contributed to the birth and development of paediatrics more than Naples. This is why it can be considered the historic capital of Italian paediatrics. Here are the main reasons: Luigi Somma was the first professor of Italian paediatrics whereas Francesco Fede was the first president of the Italian Paediatrics Association. ⋯ The first three Italian journals of paediatrics were founded in Naples as well as the journal ''La Pediatria'' which was the most distributed and long-lasting journal in this field. Moreover, Neapolitans have been the most numerous presidents of the Italian Paediatrics Association, while Rocco Jemma was the one who remained the longest in charge. ''Rocco Jemma's school'' taught not only to most professors in paediatrics who afterwards taught in most Italian universities, but also four out of five paediatricians who took charge of the position as president. The first regional department of the Italian Paediatrics Association was founded in Naples as well as the Association of Nipiology.
