To compare the incidence of gestational diabetes mellitus (GDM) in women with three or more risk factor to developing GDM supplemented with myo-inositol plus probiotics versus women care without supplementation. ⋯ Supplementation from 12-14 weeks of gestation with myo-inositol plus probiotics decrease the incidence of GDM in Mexican women.

Except for pregnant women, the management of critically ill patients with COVID-19 during the pandemic includes the standard procedures that are used for any patient that requires to be attended to at the intensive care unit, as well as limited administration of crystalloid solutions, orotracheal intubation, invasive mechanical ventilation in the event of patient clinical deterioration, and muscle relaxants continuous infusion only if necessary. Non-invasive mechanical ventilation and high-flow oxygen therapy are not recommended due to the generation of aerosol (associated with risk of viral spread among health personnel), and neither is extracorporeal membrane oxygenation or the use of steroids. So far, there is no specific antiviral treatment for patients with COVID-19, and neither are there results of controlled trials supporting the use of any.

Pregnancies resulting from assisted reproductive technologies (ART) have been documented to have a higher risk of adverse effects. ⋯ Pregnancies by ART are associated with an increased risk of obstetric and perinatal complications in comparison with spontaneous conceptions. Further research is needed to determine which aspects result in higher risk.

Breast milk is a complex biological fluid. Additionally to its nutritional impact, it contains diverse bioactive elements related to early metabolic programming and molecular structures, such as microRNA related to the epigenetic signaling process. ⋯ Apparently the origin of this microbiota is found in the entero-mammary circulation, as well as in the retrograde circulation of the skin of the mammary gland, although it is speculated in what percentage the microbiota of the oral cavity of the infant contributes in a significant way. From a functional point of view, it has been shown the transfer of this microbiota to the infant's digestive tract, which is related to better digestive tolerance, lower frequency and intensity of dysfunctionalities of the brain-intestine-microbiota axis, and improved immunity, among others.

Writing a prescription or indicating a treatment is usually the last part of medical consultation. This crucial process can be undermined by multiple factors such as limited prescriptive ability, overwork, and lack of reflection or time. Insufficient information about the patient or the treatment affects the prescriptive process and leads to errors that can be serious for patient health. The National Academy of Medicine, in line with the World Health Organization, emphasizes the relevance of making the prescriptive process a reflective exercise.
