In-hospital diagnosis delay (IHDD) of pulmonary tuberculosis (TB) has a significant impact on nosocomial TB transmission. We investigated the risk factors associated with prolonged IHDD in Taiwan, a high-resource, mid-incidence area. ⋯ To minimize IHDD, clinicians should carefully manage hospitalized patients with risk factors for prolonged delay, such as those with negative sputum smears, non-cavitary lesions on chest radiographs, admission to departments other than chest medicine/infectious diseases, exposure to fluoroquinolones before antitubercular treatment, underlying malignancy, and age > 65 years.

Cone-beam computed tomography (CBCT) can provide valuable data for root canal systems of human teeth. This study used CBCT to evaluate the number of roots and canals in mandibular first molars in Taiwanese individuals. ⋯ Our results showed a high overall incidence (32.6%) of three-rooted mandibular first molars in Taiwanese individuals. CBCT could be a valuable tool for identifying an extra distolingual root in mandibular first molars.

Marriages between Taiwanese men and immigrant women are common in Southern Taiwan. However, little is known about the adjustment of these women to life in Taiwan and their children's development as a result of cross-national marriage. This study evaluated the psychological status and adjustment of the foreign-born mothers in Taiwan, and assessed the influence of their immigrant motherhood on child development. ⋯ This study highlights the need to give continuous psychosocial support to immigrant mothers and to identify early developmental delays among their children.

End-of-life decisions are always difficult and complex, especially in the surgical setting. This study examines the epidemiology of do-not-resuscitate (DNR) orders, and the clinical factors influencing DNR consent. The impact of DNR on treatment and resource use in the surgical intensive-care unit (ICU) is also assessed. ⋯ Although DNR was common in our surgical ICU patients, this request was signed late in the ICU course, when therapeutic options had been exhausted. Early initiation of DNR discussion should be promoted to improve end-of-life care and reduce futile treatments in the ICU.

Incontinentia pigmenti is a rare, X-linked, dominant genodermatosis affecting skin, teeth, eyes, and central nervous system. Symptoms are associated with mutations in the nuclear factor-kappa B essential modulator (NEMO) gene on chromosome Xq28. Here, a subpopulation of Chinese patients with incontinentia pigmenti were examined to investigate the frequency and pattern of NEMO mutations, and to analyze their clinical features. ⋯ Mutational analysis of the NEMO gene was helpful in diagnosing incontinentia pigmenti among participants with a nearly normal phenotype or an incomplete form of the disease that only caused hyperpigmentation symptoms.
