To determine diagnostic accuracy of NMP 22 and urine cytology in the detection of transitional cell carcinoma (TCC) urinary bladder taking cystoscopy as a gold standard in patients having provisional diagnosis of bladder cancer (BC). ⋯ NMP22 is a quick, point of care test having higher sensitivity, NPV and accuracy but similar specificity and PPV to urine cytology for detection of TCC urinary bladder. Combination outperformed both in terms of sensitivity while having modest specificity.

Coexisting giant splenic artery aneurysm and non-functioning pancreatic neuroendocrine tumor.
The splenic artery aneurysm (SAA) is rare clinical entity which is the third most common intra-abdominal aneurysm. Pancreatic neuroendocrine tumors (pNETs) are rare malignancies which comprise less than 2% of all pancreatic tumors. Non-functioning pancreatic neuroendocrine tumors set forth up to 90% of all PNETs. ⋯ Distal pancreatectomy, splenic aneurysm resection and splenectomy was performed. Pathological results revealed that there was a 12 cm sized giant true splenic aneurysm and 0.7 cm sized neuroendocrine tumor in the pancreatic tissue. This manuscript is presentation of surgical approach to a case with coexistence of these two rare conditions.

To analyze the clinical effect of nasal continuous positive airway pressure (CPAP) combined with pulmonary surfactant in the treatment of neonatal respiratory distress syndrome (NRDS). ⋯ Endotracheal injection of pulmonary surfactant combined with CPAP in the treatment of NRDS can enhance the efficacy, promote the recovery of blood gas index, and reduce the parameters of mechanical ventilation and the incidence of complications, which is conducive to improving the respiratory function of the newborn. The therapy is worth application in the treatment of NRDS patients.

A large number of university teachers in Saudi Arabia comprise of expatriates. Their experiences are unique in context of the challenges and benefits of academic expatriation. The purpose of this study was to describe the experiences of expatriate university teachers in Saudi Arabia. ⋯ Expatriate faculty members described antecedents for their motivations at work. They shared their experiences regarding job adjustments, work environment and professional commitment.

Fluid overload is an independent marker for mortality in critically ill patients. Assessment of fluid status and fluid responsiveness is crucial for the management of these patients. In this study, we compared the lactate level, inferior vena cava (IVC) diameter and non-invasive cardiac output (CO) monitoring in prediction of mortality in emergency department. ⋯ Initial lactate value is a reliable parameter for prediction of mortality in non-traumatic critically ill patients. IVC diameter changes during spontaneous ventilation and non-invasive CO monitoring does not possess acceptable accuracy for prediction of mortality in these patients.
