Effectiveness of the Health Complex Model in Iranian primary health care reform: the study protocol.
Iranian traditional primary health care (PHC) system, although proven to be successful in some areas in rural populations, suffers major pitfalls in providing PHC services in urban areas especially the slum urban areas. The new government of Iran announced a health reform movement including the health reform in PHC system of Iran. The Health Complex Model (HCM) was chosen as the preferred health reform model for this purpose. ⋯ Assessing the effectiveness of the HCM, as the Iranian PHC reform initiative, could help health system policy makers for future decisions on its continuation or modification.

The association between cognitive function and objective adherence to dietary sodium guidelines in patients with heart failure.
Although cognitive impairment is common in heart failure (HF) patients, its effects on sodium adherence recommendations are unknown. ⋯ Adherence to urine sodium collection was poor, especially among those with poorer cognitive function. Sodium consumption exceeded recommended amounts and was unrelated to cognitive function. Interventions for improving sodium adherence should focus on at-risk groups (high SES and BMI) and at improving knowledge of recommended salt intake.

Development and validation of a predictive model of failed stepping-down of inhaled corticosteroids in adult asthmatics.
Asthma guidelines suggest stepping-down of inhaled corticosteroids (ICSs) when asthma is stable. ⋯ Our results suggest that comorbidity with rhinitis/rhinosinusitis and phlegm grade are imperative to predict failed stepping-down of ICSs in controlled patients with adult asthma.

Time to lack of persistence with pharmacological treatment among patients with current depressive episodes: a natural study with 1-year follow-up.
Medication nonadherence remains a big challenge for depressive patients. This study aims to assess and compare the medication persistence between unipolar depression (UD) and bipolar depression (BD). ⋯ Treatment discontinuation was frequently seen in patients with UD or BD, especially in the first 3 months of treatment. In spite of the similar pattern of medication persistence, UD and BD differ from each other in predictors of premature drop out.

Prediction of pain in orthodontic patients based on preoperative pain assessment.
To investigate whether pretreatment assessment of experimental pain can predict the level of pain after archwire placement. ⋯ A simple and noninvasive preoperative sensory test (the cold pressor test) was useful in predicting the risk of developing unbearable pain in patients after archwire placement. Self-reported pain after archwire placement decreased as individual pain tolerance increased.
