Subarachnoid phenol block for pain relief in gynecologic malignancy.
One hundred and thirty-three subarachnoid phenol blocks were performed in 90 patients with intractable pain secondary to gynecologic cancer at the University of Iowa Hospitals in the period 1961 to 1975. Excellent to moderate pain relief was obtained in 77% of 117 evaluable blocks. ⋯ Complications were temporary and occurred in 71% of the blocks, the most common being urinary and rectal incontinence. Subarachnoid phenol block is an effective method for relief of intractable pain; its use in gyneclogic oncology is discussed.

Electronic monitoring of the fetal heart rate and uterine contractions during cesarean section under balanced anesthesia.
The influence of balanced anesthesia (BA) on fetal heart rate (FHR) and uterine contraction (UC) patterns was evaluated in pregnant women undergoing emergency cesarean section. The results showed that during operative delivery under BA there was a significant decrease in the beat-to-beat variability of the FHR, most probably due to the anesthetic agent. There was a decrease in uterine contractility expressed by the significant decreases in the peak pressure as well as in the duration of the uterine contractions. Periodic changes in the FHR as decelerations and acclerations disappeared or improved mostly due to the decrease of uterine contractility.

Case ReportsThe choice of long-term anticoagulants for the pregnant patient.
The use of long-term anticoagulants in a pregnant patient poses many problems. In order to minimize the hazards for the patient and fetus the therapy must be individualized. A therapeutic regimen which calls for the use of both heparin and sodium warfarin at various intervals during gestation is discussed.

Experiences with unselected intrapartum fetal monitoring in the 3-year period from 1972-1974 are compared with those of the 3 preceding years before monitoring was instituted. The effect of monitoring all laboring patients is correlated with a significant decrease in the perinatal mortality and a significant increase in the cesarean section rate. ⋯ The advantages and disadvantages of unselected electronic monitoring are discussed. The importance of education of medical and paramedical personnel necessary for intense intrapartum care is emphasized.

Case ReportsAcquired hypertrichosis lanuginosa associated with endometrial malignancy.
A 46-year-old nulligravida complained of the recent development of an erythematous skin eruption and fine blond hair over her face. These complaints appeared to be symptoms of acquired hypertrichosis lanuginosa, which may be associated with malignancy. ⋯ Eighteen months later there was no evidence of the cancer, and the lanugo hairs had vanished. This is the first known instance of hypertrichosis lanuginosa associated with a gynecologic cancer and the first ever observed in which the lanugo hairs disappeared after cancer therapy.
