Previous reports have demonstrated that prior cardiac surgery is independently associated with in-hospital mortality after infant tracheostomy. We aimed to determine whether these infants would continue to be at increased risk for death following hospital discharge. ⋯ Prior cardiac surgery is independently associated with decreased survival within 1 y following tracheostomy. Clinicians and families of infants with prior cardiac surgery in whom tracheostomy after cardiac surgery is deemed necessary should consider this risk when planning long-term care.

Obesity reduces FVC, the most commonly used measurement of vital capacity (VC) and slow VC (SVC). It is unknown whether the difference between SVC and FVC is constant in different body mass indices (BMIs). We hypothesized that the difference between SVC and FVC increases as a function of BMI. ⋯ Our results indicate that FVC is larger than SVC in patients with low and normal BMI and no evidence of obstruction in the PFTs, whereas FVC is smaller than SVC in overweight and obese individual. Our findings add to the existing literature that use of FEV1/FVC may lead to underdiagnosis of obstructive airway disease in overweight and obese individuals.

Dieulafoy disease of the bronchus is a rare condition. A 28-y-old male patient with recurrent exacerbations of hemoptysis (producing up to 300 mL) over the previous 4 y was admitted to our hospital following an acute episode. A 3-mm sessile lesion covered with somewhat ulcerated smooth mucosa was detected at the distal end of the left main bronchus, at the entrance to the upper lobe bronchus. ⋯ In the very few reported cases of Dieulafoy disease, surgical resection and selective embolization were the treatments used. Endobronchial treatment has not been described before as a therapeutic approach. We emphasize that argon plasma coagulation can be a treatment option in such cases.

A COPD Health Management Program in a Community-Based Primary Care Setting: A Randomized Controlled Trial.
A number of effective strategies have been developed to improve the quality of life in patients with COPD. However, few have been implemented in patients with COPD at all stages in a community setting. This study evaluated the efficacy of a complex COPD health management intervention in rural communities in China. ⋯ The health management program is an effective community-based strategy for the prevention and management of COPD in China, increasing awareness and knowledge among patients and practitioners and improving management within the limitations of access to pharmacotherapy.
