Pain management in pediatric day surgery patients at The Maternal and Child Medical Research Center in Mongolia.
Pediatric day surgery shortens the hospital stay, reduces the exposure of nosocomial infections and allows for active parental participation. But pain delays the recovery and it increases the morbidity, including nausea and vomiting, and the maladaptive behavioral changes. This study was conducted to compare the effect of rectally administered paracetamol or diclofenac combined with regional nerve block with the traditional pain control method. ⋯ The pain intensity of the children who were treated with rectally administered paracetamol or diclofenac combined with regional nerve block before surgery was significantly decreased as compared to that of the children who were treated with the traditional method.

Cases series: ultrasound-guided supraclavicular block in 105 patients.
Multiple needle attempts to gain a muscle twitch or a paresthesia for a classical supraclavicular brachial plexus block can increase the risks of nerve damage or pain. The aims of this study were to obtain reliable clinical data on ultrasound-guided supraclavicular blocks, demonstrate the higher success rate and fewer complications, and design an injection method for patients whose brachial plexus can not be located. ⋯ An ultrasound-guided supraclavicular block is very effective in even patients whose brachial plexus can not be located.

The influence of age and gender on remifentanil EC(50) for preventing rocuronium induced withdrawal movements.
Rocuronium-induced withdrawal movements can be harmful to patients during the induction period. Remifentanil has been reported to reduce these movements effectively. In this study, we determined the EC(50) of remifentanil for the prevention of rocuronium induced withdrawal movements in male, female, old and child group. ⋯ The EC(50) of remifentanil for preventing rocuronium induced withdrawal movements was lowest in the elderly and higher in children than male adult patients. No difference in the EC(50) of remifentanil was seen between male and female adult patients.

Shivering associated with spinal anesthesia is uncomfortable and may interfere with monitoring. The aim of this study is to evaluate the effect of ramosetron, a serotonin-3 receptor antagonist, on the prevention of shivering during spinal anesthesia. ⋯ Compared to control, ramosetron is an effective way to prevent shivering during spinal anesthesia.

The effect of Gunn's intramuscular stimulation for postherpetic neuralgia -A report of 4 cases-.
Herpes zoster is the consequence of reactivation of latent varicella zoster virus from dorsal root ganglia. Postherpetic neuralgia (PHN) may be diagnosed when pain persists in a dermatomal pattern long after the vesicular erruption has healed. PHN is a kind of neuropathic pain. ⋯ We tried Gunn's IMS for treatment of PHN patients affecting thoracic dermatomes. As a result, the visual analogue scale (VAS) was decreased from 7-8 to 2-3 and the result were satisfactory. The purpose of this case report is to introduce the Gunn's IMS and review our experience for the treatment of PHN.
