Pseudo-aneurysm of left ventricle presenting as non-resolving chronic pericardial effusion.
Left ventricular pseudo-aneurysm is a variation of left ventricular free wall rupture, in which collection of blood occurs in pericardial sac, through ventricular wall defect. In pseudo-aneurysm, blood from defect does not fill the entire pericardial sac. Instead, because of adhesions between ventricular wall and the sac, escaping blood is isolated to a localised area between wall and pericardium. Here a case of ventricular pseudo-aneurysm of late onset which behaved as non-resolving pericardial effusion is presented.

Although exceedingly rare, fulminant hepatic failure in immunocompetent patients can develop with primary or recurrent infection due to herpes simplex virus. The diagnosis is frequently obscured by the absence of mucocutaneous involvement. Elevated transaminases with leucopenia and a relatively low bilirubin level may provide clues to the diagnosis. ⋯ Investigations revealed markedly raised transaminases and coagulopathy. Herpes simplex virus IGM (by ELISA) was positive. The immunocompetent woman was treated with acyclovir but the result was fatal.

Government of India and Stop TB Partnership strongly feel that Public Private Mix is reaquired to achieve the desired results of eliminating TB as a public health problem by 2050. IMA has accepted and adopted the DOTS strategy of RNTCP. Project Implementation Plan of IMA and GFATM is narrated for the readers to understand IMA GFATM RNTCP PPM. ⋯ In this project IMA has choosen to carry out intensified activities in five states and one union territory in India. With the idea, expected outcomes can be categorised at 3 levels--national, state and district. The individuals of IMA leadership will provide services on an honorary basis, so that the project can be best implemented in its truest sense.

A total of 105 male and 60 female patients were screened in the respiratory medicine outpatients' department, Institute of Postgraduate Medical Education & Research, Kolkata between December, 2002 and January, 2005. Chronic obstructive pulmonary disease patients were diagnosed on the basis of history and clinical examination while patients with body mass index > or =25 and otherwise disease-free were grouped as overweights. Patients suffering from other diseases like systemic hypertension, etc, were referred from other departments after proper evaluation. ⋯ In patients suffering from type 2 diabetes mellitus (n=4), post-tuberculous group (n=7), hypothyroid (n=6), collagen vascular disease group (n=6) showed restrictive spirometric pattern and the above groups including IHD patients showed significant reduction in exercise tolerance values. Some authors have stated that mixed ventilatory defect is characterised by low FEV1/FVC% pred in spirometry and low lung volumes where the lung volumes have to be ascertained by other methods but in the present investigation it was observed that mixed ventilatory defect can be estimated by spirometry; PEFR and/or FEF(25-75%). pred <70% whereas FEV1/FVC% pred is normal or supernormal. This finding is completely new one to predict mixed ventilatory defect.
