[Experience with hernioplasty after Lichtenstein in inguinal hernias].
The presented method of hernioplasty was used in 1242 patients with different types of inguinal hernias. In 428 patients plasty of the inguinal canal was performed by the classical method of Lichtenstein, in 814 patients the method was modified. ⋯ Improved results of treatment are associated by the authors with reduced ischemization of the sutured tissues with reticulate endoprosthesis. Recurrent hernias were noted in 0.2% in the main group, and 1.8% in the control group.

The authors present an experience with treatment of 99 patients with primary wounds of main vessels. The age of the patients was from 42 through 83 yeas. In 85 patients there were stab wounds, 9 patients had gunshot wounds, in 5 patients injuries of the main vessels were results of blunt trauma. ⋯ One patient died. Injuries of the large vessels of the upper and lower extremities took place in 15 patients. Nobody died.

[Injury to the liver in patients with severe combined traumas].
Experience with treatment of 220 patients with a severe combined trauma who were subjected to surgical intervention on abdominal organs was analyzed. In 164 of them (74.8%) there was a closed trauma, in 56 (26.2%) abdominal wounds were diagnosed. Injuries of the liver resulting from the closed trauma of the abdomen were revealed in 41 (25.6%) patients. For the assessment of severity of the injury and determination of the strategy of treatment the authors have developed a classification of liver injuries.

Comparative Study[Carotid endarterectomy and endovascular angioplasty with stenting in patients with atherosclerotic stenoses and occlusions of the internal carotid artery].
The authors made a comparative assessment of carotid endarterectomy and endovascular angioplasty with stenting in patients with atherosclerotic lesions of the carotid arteries. The authors consider that indications to stenting and carotid endarterectomy are identical in patients with stenose and occlusions of the carotid arteries. Contraindications to angioplasty of carotid arteries are determined. It was shown that angioplasty and stenting in atherosclerotic lesions of the carotid arteries was an effective method with a less number of complications as compared with carotid endarterectomy and are thought to be an adequate alternative to open surgical method of treatment of patients with stenoses and occlusions of the carotid arteries.

[Optimization of surgical strategy in trauma of the chest: correlation of thoracoscopy and thoracotomy].
The authors analyzed an experience with treatment of 4372 patients. Videothoracoscopy allowed the number of usual drainages of the pleural cavity to be reduced in closed trauma from 16.3% to 2.3%, in wounds--from 3.9% to 0.4%. Persistent hemo- and airstasis were obtained by coagulation of the vessels of the thoracic wall and lung, suturing lung wounds. ⋯ Conversion into thoracotomy was fulfilled in 91 (5.5%) patients. Thoracotomy was fulfilled in 344 (8%) patients with indications to operative interventions and severe condition and unstable hemodynamics. The number of thoracotomies and lethality in patients with penetrating wounds of the chest became 1.5-2 times less, was not considerably changed in closed traumas.
