Clinical Trial Controlled Clinical Trial[Oxygen saturation at rest, on exercise and during sleep in COPD patients undergoing pulmonary rehabilitation program. Two years prospective controlled study].
The role of pulmonary rehabilitation (PR) in COPD patients is well established. However, the PR programs includes physical exercise that may deteriorate chronic hypoxemia. The objective of the study was to evaluate impact of a long term PR program on oxygen saturation at rest, on exercise and during sleep in COPD patients no receiving supplemental oxygenation. ⋯ Median SaO2 at sleep did not change in PR-COPD patients (p = NS) but it decreased in C-COPD patients (p = 0.02). The study indicates that long term PR does not deteriorate the resting diurnal hypoxaemia, it prevent COPD patients from time depending exacerbation of nocturnal hypoxaemia and can improve oxygen saturation on exertion. The results could be influenced by baseline differences between PR-COPD and C-COPD patients.

Case Reports[Lung involvement in systemic lupus erythematosus (SLE)--own experience].
Pulmonary disorders in systemic lupus erythematosus are frequent and sometimes they are the first symptoms of the disease. SLE may cause a variety of clinical presentations and pathologic patterns, which can be difficult to diagnose. We observed 11 patients (9 women and 2 men) with pulmonary manifestations of SLE during last 18 years in our department. ⋯ In one case "shrinking lung" syndrome was confirmed by muscle function (diaphragm relaxation time) and lung function tests. Pulmonary hypertension confirmed by echocardiography, was associated with interstitial lung disease or vasculitis. Six cases representing different lung involvement pattern of SLE were described in details.

Case Reports[Mechanical ventilation with lower, as compared with traditional tidal volumes, for acute respiratory distress syndrome due to aspiration pneumonia].
54-years old patient with ARDS syndrome due to aspiration pneumonia is described. Mechanical ventilation with lower as compared with traditional tidal volumes in the treatment was successfully performed.

Case Reports[Pulmonary embolism in course of deep vein thrombosis of lower extremities in patient wit bilateral femoral bone fracture after car accident injuries].
The incidence of deep vein thrombosis in Western World is estimated at about 1 case/1000/year and of pulmonary embolism at 0.5 case/1000/year. Mortality in untreated pulmonary embolism is 30%. With adequate treatment (anticoagulation) it can be decreased to 2-8%. ⋯ When surgery has to be performed in patient with high risk of pulmonary embolism, anticoagulant prophylaxis should be performed. Venous thromboembolic disease is the most important cause of mortality in trauma patients. We present a case of 46 years old man with bilateral fracture of femoral bone after car accident injury in whom signs of deep venous thrombosis and pulmonary embolism were observed despite the use of anticoagulant prophylaxis.

Pulmonary hyalinizing granulomata are unusual, noninfectious lesions of the lung of uncertain etiology. The pathologic entity may represent a peculiar form of pulmonary immune reaction. We report the history of a 23-year-old woman who had nodular lesions in the right lung without any other abnormalities. ⋯ Remaining 3 nodules were also resected. The pulmonary hyalinizing granuloma was recognised in the histological examination. Follow-up after 12 months didn't reveal recurrence.
