Trazodone Effects on Obstructive Sleep Apnea and Non-REM Arousal Threshold.
A low respiratory arousal threshold is a physiological trait involved in obstructive sleep apnea (OSA) pathogenesis. Trazodone may increase arousal threshold without compromising upper airway muscles, which should improve OSA. ⋯ These findings suggest that trazodone could be effective therapy for patients with OSA without worsening hypoxemia. Future studies should focus on underlying mechanisms and combination therapies to eliminate OSA. Clinical trial registered with www.clinicaltrials.gov (NCT 01817907).

Enterovirus D68 (EV-D68), a member of the Picornaviridae family, was first identified in 1962 and is part of a group of small, nonenveloped RNA viruses. As a family, these viruses are among the most common causes of disease among humans. However, outbreaks of disease attributable to EV-D68 have been rarely reported in the previous 4 decades. ⋯ Between mid-August and December 18, 2014, confirmed cases of lower respiratory tract infection caused by EV-D68 were reported in 1,152 people in 49 states and the District of Columbia. A focused review of EV-D68 respiratory disease and clinical highlights from the 2014 U. S. outbreak are presented here.

Impact of relative contraindications to home management in emergency department patients with low-risk pulmonary embolism.
Studies of adults presenting to the emergency department (ED) with acute pulmonary embolism (PE) suggest that those who are low risk on the PE Severity Index (classes I and II) can be managed safely without hospitalization. However, the impact of relative contraindications to home management on outcomes has not been described. ⋯ Nearly two-thirds of adults presenting to the ED with low-risk PE were potentially eligible for outpatient therapy. Relative contraindications to outpatient management were associated with an increased frequency of adverse events at 30 days among adults with low-risk PE.

Neuromuscular blockade improves first-attempt success for intubation in the intensive care unit. A propensity matched analysis.
The use of neuromuscular blocking agents (NMBAs) has been shown to be valuable in improving successful tracheal intubation in the operating room and emergency department. However, data on NMBA use in critically ill intensive care unit (ICU) patients are lacking. Furthermore, there are no data on NMBA use with video laryngoscopy. ⋯ After controlling for potential confounders, this propensity-adjusted analysis demonstrates improved odds of FAS at intubation in the ICU with the use of an NMBA. This improvement in FAS is seen even with the use of a video laryngoscope.

Volatile organic compounds present in the exhaled breath have shown promise as biomarkers of lung cancer. Advances in colorimetric sensor array technology, breath collection methods, and clinical phenotyping may lead to the development of a more accurate breath biomarker. ⋯ The colorimetric sensor array signature of exhaled breath volatile organic compounds was capable of distinguishing patients with lung cancer from clinically relevant control subjects in a discovery level trial. The incorporation of clinical phenotypes into the further development of this biomarker may optimize its accuracy.
