An incidence of sepsis, septic shock, systemic inflammatory response syndrome (SIRS) and multiple organ dysfunction syndrome (MODS) is still actual clinical problem. Inducing factors and clinical pictures are similar to recently reported data from comparable populations in North America, Europe and Australia. ⋯ Applied intensive therapy including new generations of antibiotics gives an increase in clinical recovery. However, a hospital mortality of sepsis patients is over 30%.

Polish ethic committees should have to work together in order to maintain and develop high quality standards in the protection of human subjects. Exchanging knowledge, know-how and information polish ethic committees should have to implement standard operating procedures. Procedures should improve quality and proficiency of all types of ethic committee's activities. ⋯ The opinions of experts who have been reviewing medical research projects for several years may prove to be especially valuable in this setting. Governmental initiatives and creation of forum for polish ethic committees are essential in the effective standardisation, coordination and implementation of procedures in regional ethic committees. These projects need support via public funding from our authorities.

Since the nineties, while both pain and stress reactions in newborn were revealed, the period of opioid analgesia as well as benzodwuazepin sedation was initiated. Midazolam is the drug which is particularly willingly used in neonatal intensive care units. ⋯ In this paper the results of pharmacokinetics studies and studies concerning the adverse effects of midazolam among low birth weight newborn are collected and the current opinion of experts dedicates regarding analgetic and sedative procedures in neonatal period are presented. On the basis of the results of the previous studies, it seems reasonable to be more critical of our sedation approach with midazolam and pay more attention to appropriate administration of midazolam in neonatal period.

Clinical Trial[Effectiveness of Broncho-Vaxom in prevention of recurrent upper respiratory tract infection in children].
Evaluation of effectiveness of immunostimulating treatment with Broncho-Vaxom and his influence on concentration of proinflammatory cytokines in children with recurrent upper respiratory tract infection. ⋯ The treatment with Broncho-Vaxom shows the high effectiveness in prevention of recurrent upper respiratory tract infection in children as well as the influences of profitable changes in profile of serum cytokines which can impact on limitation the inflammation process in the upper respiratory tract.

Review[Role of procalcitonin concentration in clinical diagnosis].
Procalcytonin (PCT) is a prohormon of calcitonin. It is made in thyroid structures. Level of procalcitonin is related from actual calcium concentration in blood. It is shown that procalcitonin level increases in bacterial, fungus and parasite infections, in systematic inflammatory response syndrome (SIRS), burn and multiple organ dysfunction syndrome (MODS). We wanted to conclude the actual knowledge about the role of procalcitonin used in acute infection. Level of PCT is compatible with the etiology of infection. The procalcitonin is a fast infection marker with a longer semi-loose time than CRP. A mechanism of secretion of procalcitonin to blood circulation is still enigmatic. The determination method (immunoluminometric assay) is easy and inexpensive and results are credible and can be used to differentiating and moni-toring of disease. ⋯ It is possible that PCT could be an important marker in fast diagnosis and differentiating of bacterial infections, also could be used in treatment monitoring and prognosis.
