Randomized Controlled Trial Clinical Trial[Captopril in the treatment of acute myocardial infarction. Hypotension as an important clinical problem].
The aim of study was to analyze the effect of captopril (C) on blood pressure in patients with acute myocardial infarction (AMI) during 1 year treatment. Patients less then 70 years old with systolic blood pressure (sBP) > or = 100 mm Hg were qualified to the study. Administration of C was started during the first 4 days of AMI (mean 21 +/- 24 h). 50 pts treated with C and control group of 43 pts were finally analyzed. ⋯ In conclusion, hypotension seems to be quite often encountered during captopril therapy of early phase of AMI. It appears not only after the first but also after the following doses. The initial dose of 3.125 mg seems to be safe and sufficient to assess its hypotensive action.

Case Reports[Vasculitis of cerebral vessels, probable cause of neurological complications in a child with Schonlein-Henoch purpura].
We report 6-years old boy with Schönlein-Henoch purpura who presented neurologic manifestations: depressed state generalised convulsions, and cortical blindness. Sequential magnetic resonance imaging showed bilateral cerebral ischemic lesions in the cortex and white matter of parieto-occipital lobes caused by vasculitis.

Clinical Trial[Indirect evaluation of respiratory muscle strength with the help of markers of maximal inspiratory and expiratory pressure in the mouth of healthy individuals].
The aim of the study was the evaluation of respiratory muscle strength by measurement of maximal inspiratory (PImax) and expiratory (PEmax) pressures values. Results for 166 clinically normal subjects (79 female and 87 male) were obtained. The results were as follow: in woman PImax ranged 38-104 cm H2O, average 60 cm H2O, PEmax ranged 46-140 cm H2O average 87.5 cm H2O, in men PImax was 40-120 cm H2O, average 73.2 cm H2O, PEmax ranged 46-140 cm H2O, average 115.9 cm H2O. ⋯ The comparison of results of present study with those obtained in former studies reveals important differences of norms for different populations. The normal values of maximal inspiratory and expiratory pressures in the mouth (PImax, PEmax) should be qualified individually for studied population. The normal values recommended by producers of medical equipment might be inadequate for studied population.

Review[Sepsis--continual pathogenetic, diagnostic and therapeutic problem].
Sepsis is defined as systemic inflammatory response syndrome (SIRS) caused by infection. Massive activation of humoral and cellular mechanisms of host defense is present and can lead to a multiple organ dysfunction syndrome. Sepsis is good example, that inflammation binds with considerable energetic and metabolic effort of organism i.e. demand for oxygen, protein and microelements. ⋯ Precious advice in selection of antibiotic were given. Advantage penicillins and karbapenems over cephalosporins result from binding to PBP 2 or 1, considerably smaller release of free endotoxin and smaller induction Jarish-Heixhaimer's reaction. The side effect stimulates inflammation and deepens SIRS by release toxins and increase of TNF-alpha.

[Results of coronary artery bypass grafting in patients with impaired left ventricular systolic function].
The purpose of this study was to examine the early and late results of CABG in patients with left ventricular ejection fraction (LVEF) lower than 40%. This study comprised 128 patients (114 male and 14 female) at age from 41 to 75 years (mean 58.3 +/- 7.9). Before operation 115 patients (89.8%) were in CCS class III or IV. ⋯ Patients with LVEF < 40% are at higher operative risk because of often postoperative low output syndrome. Low output syndrome can by successfully treated with IABP. CABG significantly improves circulatory sufficiency in patients with LVEF < 40%.
