Case Reports[Acrodermatitis chronica atrophicans in the course of old Lyme disease].
Lyme borreliosis is chronic spirochetal infection with phasic course. Lyme disease can be recognised as early or late stage of disease. A case report of patient with acrodermatitis chronica atrophicans in the course of chronic (late) Lyme borreliosis is presented. As it is described, the lack of uncognition of infection of B. burgdorferi and typical signs of erythema migrans as well as a lack of proper therapy in this time may be a reason of passing into chronic phase of the disease like ACA and Lyme arthritis.

Empathy is an expected attribute improving doctor-patient relationship. It also helps to understand psychological problems of patients. This paper describes a research study on empathy in physicians of different specializations. ⋯ Comparing them to internists and gynaecologists, the difference is of statistical significance. The highest level of cognitive empathy characterizes in order: paediatricians, psychiatrists and family doctors. Comparing to other specializations, internal medicine doctors and gynaecologists proved to be physicians of the lowest cognitive empathy.

Comparative Study Clinical Trial Controlled Clinical Trial[Comparison of efficacy, safety and cost-effectiveness of intravenous versus oral propafenone in paroxysmal atrial fibrillation].
The purpose of this study was to investigate the efficacy, safety and cost-effectiveness of intravenous and oral propafenone in the conversion of paroxysmal atrial fibrillation propafenone to sinus rhythm. We analysed two groups of 100 consecutive patients (pts) treated because of propafenone with duration < 48 h. The first group was treated with intravenous PFN (bolus of 70 to 140 mg) and the second group was treated with oral PFN (300 to 600 mg). ⋯ Conversion to sinus rhythm was achieved in 64 (64%) pts who received i.v. propafenone and in 77 (77%) who received oral propafenone (p < 0.05). We conclude that oral and intravenous propafenone is safe in the termination of propafenone. Oral route of administration appears to be superior to intravenous because of greater efficacy and cost-effectiveness.

This paper reviews the biochemical and physiological data underlying hyperhomocysteinemia. Elevation of plasma homocysteine arises from disrupted of its metabolism (remethylation to methionine and transsulfuration to cystathionine) and is a function of a complex interaction between multiple genetic and environmental factors. Hyperhomocysteinemia, a conditions that recent epidemiological studies have shown to be associated with an increased risk of vascular diseases, may be a equal importance to hypercholesterolemia, hypertension and smoking in etiology of atherosclerosis. Based on the role of folic acid, vitamin B12 and B6 in homocysteine metabolism supplementation with these vitamins in treatment of hyperhomocysteinemia both in cardiovascular diseases and neural tube defects is reviewed.

Clinical Trial Controlled Clinical Trial[Analysis of voice function disorders using videostroboscopic techniques].
Clinical applications of videostroboscope in phoniatrics were investigated. Altogether 79 patients with functional lesions of the larynx and 20 with normal were examined. Measurements of vibration, amplitude, frequency and mucosal wave of vocal cords yielded cords objective information about vibration in the functional dysphonia. Videolaryngostroboscope can help identify the kind of functional diseases of the larynx.
