[Clinical application of high frequency jet ventilation in cryotherapy for tracheo-bronchial carcinoma via rigid bronchoscopy].
Objective: To analyze the clinical applications of high frequency jet ventilation(HFJV) in cryotherapy of the trachea and bronchial neoplasms by the rigid bronchoscope. Methods: The clinical data of 35 patients who were treated with tracheal neoplasms cryotherapy by the rigid bronchoscopy under HFJV were collected in China-Japan Friendship Hospital from August 2008 to February 2015. Under general anesthesia, HFJV was used in all patients. ⋯ The patient has a serious accumulation of carbon dioxide (CO(2)) whose partial pressure of carbon dioxide in the artery (PaCO(2)) was up to 71 mmHg(1 mmHg=0.133 kPa). Other patients had stable hemodynamics and no severe CO(2) accumulation. Conclusion: High frequency jet ventilation can provide satisfactory ventilation effect in cryotherapy of the trachea and bronchial end-stage neoplasms by the rigid bronchoscope.

[Effect of N-acetyl-L-cysteine on vascular heterogeneity of microcirculation in endotoxemia rabbits and its mechanism].
Objective: To study the effect of N-acetyl-L-cysteine (NAC) on vascular heterogeneity in microcirculation dysfunction caused by endotoxemia and its mechanism. Methods: Eighteen Japanese big ear rabbits were divided into three groups: Control group, Lipopolysaccharides (LPS) group and LPS+ NAC group. A dose of 2 mg/kg LPS was injected to set up endotoxemia rabbit models. ⋯ The arterial-venous difference of S-nitrosohemoglobin content in Control group and LPS+ NAC group were higher than LPS group [(41±6) vs (27±5) nmol/L, P=0.002; (36±5) vs (27±5) nmol/L, P=0.010]. Conclusions: In endotoxemia rabbits, NAC can improve the microcirculation dysfunction, especially vascular heterogeneity. It indicated that NAC could improve the ability of S-nitrosohemoglobin-mediated nitric oxide release from red blood cells.

[Role and mechanism of lipopolysaccharide induced exosome in the pathogenesis of acute lung injury].
Objective: To explore the effect and mechanism of exosome derived from lipopolysaccharide (LPS)-induced mouse macrophage (RAW264.7) on acute lung injury. Methods: RAW264.7 were cultured in vitro and divided into 2 groups: control group and LPS-induced group. The exosomes were extracted from the two groups of cell supernatant by ultracentrifugation and classified into 2 groups: C-EXO group and LPS-EXO group. ⋯ While the tissue protein levels of Occludin showed the opposite phenomenon (0.251±0.021 and 0.862±0.029, 0.453±0.013; all P<0.05). In vitro, Compared with the C-control group and the EXO-control-induced group, the expression levels of IL-1β and TNF-α increased significantly in the EXO-LPS-induced group (0.900±0.033 and 0.320±0.030, 0.661±0.028; 0.739±0.045 and 0.151±0.024, 0.360±0.037; all P<0.05). whereas the protein levels of Occludin expression were reversed in MLE-12 (0.585±0.082 and 0.941±0.090, 0.732±0.083; all P<0.05). Conclusion: Exosomes derived from LPS-induced RAW264.7 can induced the acute lung injury by affecting barrier function, which probably is related to the low degree of Occludin in alveolar type Ⅱ epithelial cells.

[Establishment of a canine slow transit constipation model and evalution of defecation, gastrointestinal transit and pathological sections].
Objective: To establish a canine model of slow transit constipation (STC), and to test the changes in defecation, gastrointestinal transit time and pathology sections. Methods: Baseline information was measured in 8 beagle dogs, and these dogs were randomly divided into the control group and the model group. The dogs in model group were given a diet of canned meat, as well as a combination of compound diphenoxylate and alosetron hydrochloride for 5 weeks. ⋯ After 5 weeks of intervention, in the model group, the myenteric neurons and interstitial cells of Cajal showed damage such as swelling of mitochondria under electron microscopy, and both of the PGP9.5 and synaptophysin integrated option density of rectosigmoid colon were decreased (t=3.471, P=0.013; t=2.506, P=0.046)under immunohistochemistry. The c-kit integrated option density showed no statistically significant differences between two groups(t=1.709, P=0.138). Conclusions: The canine model of STC which was consistent with clinical symptoms and pathological changes was successfully established, and it can be used to observe and evaluate the therapeutic effect of electrical stimulation, surgery and so on.

[Similar coronal curvature may not represent the same 3-dimensional deformity in adolescent idiopathic scoliosis: a matched-pair study using EOS imaging system].
Objective: To analyze the preoperative axial plane and the surgical outcomes of the Lenke type 1A patients with adolescent idiopathic scoliosis (AIS) whose coronal curve type was matched but thoracic kyphosis (TK) was different. Methods: This study retrospectively reviewed a series of Lenke type 1A female AIS patients who underwent corrective surgery in the Department of Spine Surgery of Nanjing Drum Tower Hospital from May to August 2017. After matched with the Cobb angle of the main thoracic curve, the apical vertebral, the vertebra number included in the curve, the lumber modifier in the Lenke classification and Risser sign, 12 pairs of AIS patients, whose coronal curve was matched but thoracic kyphosis was different, were included in this study. ⋯ The lumbar Cobb angle correction rate was significantly greater in patients with thoracic and lumbar curve rotating in the same direction than that in the opposite direction (81.1% vs 61.9%, t=4.24, P=0.005). Conclusions: It indicated that when the coronal deformity is matched, the MTR of the patients with thoracic hypokyphosis is significantly larger than that in the patients with normal thoracic kyphosis. The direction of the thoracic and lumbar curve rotation is required to be well concerned in the preoperative surgical planning.
