The internal jugular vein cannulation is a commonly used route for access to the central venous system for therapeutic and diagnostic purposes. Although the great majority of this venous puncture are successfully performed using anatomic landmark technique, serious complication such as pneumothorax, arterial puncture or hemothorax can occur during this procedure, particularly under difficult clinical condition. ⋯ Ultrasound guidance reduces the number of passes needed to puncture the vein, increases the success rate of venous puncture, and minimizes complications. Ultrasound guidance is usually successful in allowing performance of internal jugular vein cannulation when landmark technique fail.

High doses of chemo- and radiotherapy, while theoretically more effective, are accompanied by severe toxicities and normal cells damage. This may have important implications for patients' management and can result in significant morbidity. Therefore, it is imperative to protect normal tissues from both the early and late damage caused by high doses of radiation and chemotherapy. ⋯ Since 1986, initial trials with Amifostine in patients with diverse advanced neoplasms treated by chemotherapy and radiation therapy have been performed. Published data suggest that pretreatment with Amifostine significantly decreases hematologic, mucosal and renal toxicity as well as the frequency of neuropathy. Also, significant reduction in the frequency of hospitalization, mean hospital stay, and the number of days on antibiotics was observed after application of Amifostine.

Determination of the carboxyhemoglobin (COHb), methemoglobin (MetHb), and blood lactate concentrations is known as important contributive in evaluation of oxygen deficiency in people from urban populations. The methemoglobinemia is usually caused by unfavourable impact of NOx. A quantification of COHb allows, to some extend, to evaluate the threat of population with carbon monoxide (CO). ⋯ No unfavourable impact of Nox reflected in methemoglobinemia was noted in any examined population. The determination of COHb levels is helpful in diagnosis of acute CO poisoning. The measuring of the blood lactate concentration allows to detect the oxygen insufficiency in acute CO poisoning and in states of long-term exposure to CO.

[Appendiceal abscess in the third gestational trimester of pregnancy, complications pre and postoperatively].
Delayed surgical intervention connected with misdiagnosis of preterm labour and urinary tract infection caused in gravida 3 in 34th gestational week appendiceal abscess, septic shock, stillbirth by cesarean section, necessity of hysterectomy, recidivism of multi peritoneal and pleural abscesses. Although the patient was rescued the retrospective pro memoria considerations of our procedure are regarded.
