[Arthroscopic capsular release in frozen shoulder syndrome].
PURPOSE OF THE STUDY To evaluate the results of arthroscopic capsular release for the treatment of severe frozen shoulder syndrome. MATERIAL Between 2006 and 2008, 27 patients with severe frozen shoulder syndrome were treated by arthroscopic capsular release. The average age of the patients was 54 years (range, 34 to 75), 15 were men and 12 were women. ⋯ It provides marked improvement in the range of motion and is associated with a minimum of post-operative complications. However, some patients may complain of persisting discomfort in the joint treated. Key words: frozen shoulder, arthroscopy, capsular release.

[Long-term results of the CLS acetabular cup in primary total hip replacement].
PURPOSE OF THE STUDY To evaluate the results of primary total hip replacement with the CLS acetabular cup at a minimum of 15 years of follow- up. MATERIAL AND METHODS A total of 105 patients, with 112 hips, undergoing hip arthroplasty with the use of CLS acetabular cup between 1991 and 1993 were evaluated. The group comprised of 33 men and 72 women with an average age of 51.3 years (range, 28 to 66) at the time of surgery. ⋯ No increase in fractures of the shell, migration or radiolucent lines, or in bone rarefaction or osteolysis was recorded with time. The reliability of this implant has been recently increased with the use of cross-linked polyethylene (Durasul). Key words: primary total hip replacement, uncemented cup.

[Hyperextension trauma in patients with cervical spondylosis].
PURPOSE OF THE STUDY To evaluate retrospectively a group of patients with hyperextension injury to the cervical spine who were treated at the Department of Spinal Surgery of the University Hospital in Motol, Prague, between 2003 and 2006. MATERIAL The group comprised 22 patients, 17 men (77 %) and five women (23 %) in the age range of 35 to 81 years, with an average of 59.5 years. All patients had, in association with the injury, neurological deficit of varying degree. ⋯ A correct algorithm of examination will result in good treatment outcomes. However, these injuries require a therapy that is long-lasting and difficult, with a need for cooperation of anaesthesiologists, spinal surgeons, physical therapists and, last but not least, psychologists. PURPOSE OF THE STUDY Surgery for the nonunion or malunion of pelvic fractures is not common even at specialised departments. This article presents the authors experience with this procedure, completed with case studies and a review of relevant literature. MATERIAL AND METHODS From 1998 to 2007, a total of 359 patients underwent surgery for pelvic trauma. ⋯ CONCLUSIONS Early surgery of the pelvic trauma enables an adequate restoration of pelvic anatomy and provides conditions for good and reliable stability of both the posterior and anterior pelvic segments. Late repairs of nonunions or malunions are demanding and associated with a high risk of serious complications, often with long-term sequelae. Key words: pelvic fracture, malunion, nonunion.

[Regional anaesthesia of the foot achieved from two cutaneous points of injection: an anatomical study].
PURPOSE OF THE STUDY Regional anaesthesia for the lower extremity distal to the ankle joint, knows as anaesthetic ankle block or foot block, involves a series of injections of local anaesthetic to block the peripheral nerves that supply innervation to the foot. Since the tibial nerve block is not always effective, the aim of this study was to design a modified technique of anaesthetic application. MATERIAL The study was carried out on 30 human cadavers provided by the Institute of Anatomy, 1st Faculty of Medicine, Charles University in Prague, and included data on a total of 60 lower extremities. ⋯ CONCLUSIONS The results of this anatomical study provided a basis for a modified technique of regional anaesthesia of the foot. Its major clinical attributes involve reduced necessity to manipulate with the patient, and sensory and motor blockage of all nerves supplying innervation to the foot from only two points of injection.
