Clinical Trial[Evaluation of cases of metastatic liver tumors resected following intra-arterial infusion chemotherapy].
We encountered four cases whose unresectable metastatic liver tumors could be resected by operation after intra-arterial infusion chemotherapy using an implantable reservoir. Metastatic lesions were thus markedly reduced and localized. Three cases were metastases of colorectal cancer and one of gastric cancer. ⋯ Our results indicate that this chemotherapy could be a neoadjuvant chemotherapy for unresectable metastatic liver tumors which may become resectable after this chemotherapy as seen in our cases. However, lung metastases and peritoneal dissemination influenced the prognosis of our cases. Treatment of extra-hepatic recurrence seemed important to improve the prognosis of these patients.

Clinical Trial[Clinical evaluation of granisetron injection against nausea and vomiting induced by anticancer drugs including cisplatin].
The antiemetic effect and safety of granisetron injection on nausea and vomiting induced by anticancer drugs were studied in the patients treated with anticancer drugs including 50 mg/m2 or more of cisplatin (CDDP). Granisetron is already on the market, and drip infusion of granisetron at 40 micrograms/kg has been used widely in clinical practice. In this clinical investigation, a simpler administration method of its slow (30 to 60 seconds) intravenous injection at 40 micrograms/kg just before CDDP administration was used. ⋯ Neither adverse experience nor abnormal laboratory test value was reported. In the usefulness rating, the study medication was assessed as "extremely useful" or "useful" in 16 out of 22 patients (72.7%). The above results have shown that the slow intravenous injection of granisetron has an excellent antiemetic effect on nausea and vomiting induced by anticancer drugs including CDDP and a high degree of safety.

[The accuracy and clinical issue of preoperative diagnosis of gastric cancer].
We discussed the accuracy and clinical issue of preoperative diagnosis for 764 patients with gastric cancer from 1987 to 1990. The accuracy of diagnosis, location, tumor size, wall invasion, involved lymph node were 81.8, 80.5, 62.2, 82.5% respectively. ⋯ Especially in the case of early gastric cancer, we missed diagnosis of wall invasion of the regions with clinicopathological characteristics as follows: depressed type, undifferentiated type and ulceration in lesions. Now it is difficult to diagnosis wall invasion before surgery, so we have to be careful to determine the indication of endoscopic resection or limited operation for early gastric cancer, especially with clinicopathologic characteristics stated as above.

[Early detection of breast cancer by mass screening using mammography].
A mass screening trial for breast cancer using mammography was carried out in Tokushima Prefecture, either itinerant screening in the communities or center screening at the workplace. Craniocaudal and lateral oblique imagings of the breast using two-view film mammography were performed at the first stage of screening. In addition, physical examination of the breast was also performed on subjects independently. ⋯ The number of clinical stage for cases with breast cancer detected by these screening were 5 cases in TIS and 3 in stage I. Thus, all cases were patients with early-stage breast cancer. From these results, mass screening using mammography should be mandatory for elderly women (over 50 years) with asymptomatic breasts.

Case Reports[A case of malignant lymphoma of the thyroid accompanied by interstitial pneumonitis during radiation therapy after systemic chemotherapy].
We reported a case of malignant lymphoma of the thyroid accompanied by interstitial pneumonitis during radiation therapy after systemic chemotherapy. Thyroid malignant lymphoma is a rare malignant tumor, which usually responds well to irradiation and systemic chemotherapy. ⋯ The radiation field included the neck and upper mediastinum. Chest X-ray showed multiple patchy shadows in both the middle and lower lung field, and it changed to contraction shadows after steroid pulse therapy. 67Ga scintigram showed multifocal increase of uptake, when chest X-ray patchy shadows were seen only in the middle and lower lung fields. 67Ga scintigraphy and CT were considered useful for evaluation of drug-induced interstitial pneumonitis.
