Randomized Controlled Trial Multicenter StudyEfficacy and safety of intravenous ulinastatin versus placebo along with standard supportive care in subjects with mild or severe acute pancreatitis.
Ulinastatin is reported to inhibit pro-inflammatory markers and also inhibits coagulation and fibrinolysis. The drug is available in East Asia for the treatment of acute pancreatitis. ⋯ Ulinastatin prevents new organ dysfunction and reduces mortality in subjects with severe pancreatitis.

Status epilepticus (SE) is a neurological emergency resulting from prolonged Clinical or electroencephalographic seizure activity. Refractory SE refers to the persistence of seizure activity despite the initiation of first- and second-line anticonvulsant therapy. Sinister outcomes are often attributed to the etiology of SE. ⋯ The extent of patient support has to be augmentative to the degree of encephalopathy/coma and impairment of vital functions. Potential interactions of anticonvulsant drugs with other co-medications need to be considered during the course of treatment. This review discusses the existing literature on the epidemiological aspects, clinical approach, treatment and prognostication of SE.

Case ReportsMiliary sarcoidosis with secondary Sjogren's syndrome.
We report a case of miliary sarcoidosis with secondary Sjogren's in a 45-year-old male who presented with symptoms of sicca syndrome in the form of dryness of eyes and mouth with parotid swelling. Computed tomography thorax showed mediastinal and hilar lymphadenopathy, bilateral miliary opacities in lung parenchyma. Whole body FDG PET/CT showed involvement of both parotids, liver, diffuse uptake in lungs, mediastinal and retroperitoneal lymph nodes. Patient is on treatment with prednisolone and has responded well.

We report here a case of Shapiro syndrome who presented with episodic generalized sweating, hypotension, and hypothermia. Brain magnetic resonance imaging demonstrated corpus callosum agenesis with colpocephaly. Patient was treated with Clonidine and Propranolol. This case is being reported here because only a few cases of Shapiro Syndrome are reported in world literature.
