Case ReportsSpontaneous spinal epidural haematoma during pregnancy.
Spontaneous spinal epidural haematoma (SSEH) is rare in pregnancy and only five cases have been reported. We present a case of a 25-year-old female G1P0 (gravida one, para zero) who at 31 weeks of gestation developed progressive descending quadriplegia and numbness upto the level of C4. ⋯ The patient's neurological function subsequently improved. The diagnosis and management of spontaneous epidural haematoma in pregnancy is presented with a review of the literature.

The link between tuberculosis (TB) and diabetes mellitus (DM) has occupied the center stage of discussion. Experts have raised concern about the merging epidemics of tuberculosis and diabetes particularly in the low to medium income countries like India and China that have the highest burden of TB in the world, and are experiencing the fastest increase in the prevalence of DM. ⋯ This review looks at the link between these two merging epidemics. We discuss the epidemiology, clinical features, microbiology and radiology, and management and treatment outcomes of patients with tuberculosis and diabetes mellitus.

Air travel is implicated as a predisposing factor for thromboembolism, which at times can have catastrophic consequences. We present three cases who developed deep vein thrombosis (DVT) and subsequent pulmonary thromboembolism (PTE) after transatlantic air travel.

A retrospective analysis of a rural set up experience with special reference to dobutamine in prazosin-resistant scorpion sting cases.
Incidence of scorpion sting is high in west coast of Maharashtra. Scorpion sting gives rise to cardiotoxicity and it is our observation that in spite of using prazosin in initial phases, patients still developed severe cardiotoxicity and dobutamine was effective in reducing the mortality in these patients. So we want to convey this message to many of our doctor friends who practice in rural area. ⋯ In initial phases of autonomic storm i.e. accelerated hypertension one can use prazosin but monitor the patient for left ventricular failure and clinically if patient develops LVF add dobutamine to prevent death. Addition of Dobutamine can be life-saving.
