Internists are frequently consulted to perform preoperative risk assessments on patient undergoing non-cardiac surgery. Physicians need to assess the cardiac risk associated with the surgery, identify the patient's unique risk profile, recommend appropriate preoperative testing and make suitable recommendations. ⋯ Effective preoperative recommendation includes an assessment of risk (cardiac and non-cardiac), recommendations on medication adjustment, thrombo-embolic prophylaxis, and use of perioperative beta blockers. Prompt communication with the surgeon often enhances the quality of medical consultation and ensures compliance.

Case ReportsBilateral angle closure glaucoma following snake bite.
Snake venoms are complex heterogenous poisons with multiple effects. Ocular complications with snake bite are rare. This report describes two cases which developed bilateral acute angle closure glaucoma following snake bite. There was complete recovery in the two patients following timely diagnosis and treatment.

Case ReportsBilateral internuclear ophthalmoplegia following head injury.
A 40-year-old female after a closed head injury presented with bilateral internuclear ophthalmoplegia. Attempted convergence was abnormal and MRI revealed the focal hemorrhage in the medial longitudinal fasciculus region by showing bright signal in the a pontomesencephalic region in the midline on both T1 and T2 weighted images. ⋯ The medial longitudinal fasciculus, which is believed to be lesioned in cases of internuclear ophthalmoplegia, is an unusual and rare finding, particularly in patients victims of head injury without further neurological signs. Isolated internuclear ophthalmoplegia should be considered in the differential diagnosis when one encounters an adduction deficit in a patient suffering head injury.
