[Management of bleeding esophageal varices in public and private institutions in Chile].
The better treatment modalities for bleeding esophageal varices have improved the prognosis of cirrhosis. ⋯ Public health institutions have poor access to adequate diagnostic and treatment methods for esophageal bleeding. The primary and secondary prophylaxis of esophageal varices must be improved in both types of institutions.

[Prostacyclin production in internal mammary ano radial arteries and saphenous veins of diabetic and non diabetic subjects].
Myocardial revascularization surgery has used several vessels as coronary grafts including internal mammary and radial arteries which have a better prognosis than saphenous vein. Their long-term patency has been associated with the release of endothelium vasodilator and anti-aggregating products such as prostacyclin. Diabetes induces endothelial dysfunction and a high number of diabetics require revascularization. ⋯ The radial artery appears as the best replacement vessel for coronary surgery in diabetics. Its favorable biochemical profile and potential lower long-term occlusion rate may be relevant for a better prognosis of myocardial revascularization in these patients.

Randomized Controlled Trial Comparative Study[Protocol based sedation versus conventional treatment in critically ill patients on mechanical ventilation].
Sedatives and analgesic drugs give comfort and allow adequate respiratory support to critically ill patients in mechanical ventilation (MV). Its improper use may increase the duration of MV. Clinical guidelines suggest implementation of protocols, however this is seldom done in clinical practice. ⋯ The implementation of sedation protocol applied by nurses improved the quality of sedation and reduced the doses of Midazolam in mechanically ventilated patients.

Case Reports[Atrial thrombus entrapped in a patent foramen oval. Report of one case].
We report a 63 year-old female with a pulmonary embolism in whom echocardiography revealed the presence of right heart thrombus. A section of this thrombus was entrapped in a patent foramen oval and floating in both atria. This rare situation, named impending paradoxical embolism, prompted us to perform a surgical intervention, removing the thrombus and repairing the foramen ovale.
