Review[Hemodynamic impact of positive end expiratory pressure (PEEP) during severe respiratory failure: present view].
Bedside evaluation of pulmonary mechanics and thoracic computed axial tomography have changed the ventilatory management of patients suffering an acute respiratory failure caused by adult respiratory distress syndrome (ARDS). Mortality has been reduced limiting tidal volumes, which avoids alveolar overdistention and by the use of positive end expiratory pressure (PEEP), that reduces the damage caused by cyclical alveolar collapse-reopening. ⋯ Usually, it is not easy to achieve an equilibrium between an optimal lung recruitment without producing alveolar overdistention and hemodynamic adverse effects such as hypotension and reduction of cardiac output. This paper reviews the interactions between heart and lung.

Community-acquired pneumonia (CAP) is a serious health problem in Chile. ⋯ The features of community acquired pneumonia of these patients are similar to those reported abroad.

[Fontan procedure: early and mid-term results with total cavopulmonary anastomosis].
During the last five years, 65 patients with univentricular heart have been treated surgically in our institution, according to a protocol of staged operations that have been previously reported. ⋯ Our current results are comparable with those of larger series. Patients reach good FC and mid-term survival, irrespective of type of single ventricle or the surgical strategy.

Case Reports[Mesenteric acute ischemia caused by spontaneous and isolated dissection of the superior mesenteric artery].
Spontaneous dissection of the superior mesenteric artery is an unusual cause of mesenteric ischemia. Diagnosis can be based on findings of the belicoidal computed tomography or selective angiography of the superior mesenteric artery. ⋯ Isolated superior mesenteric artery dissection was confirmed by angiography. Both patients were treated with anticoagulation, conservative measures and recovered uneventfully.

Editorial[Update of authorship criteria in biomedical articles].
In the year 2000, a previous Editorial in this journal reviewed the criteria for authorship as defined by experienced authors and editors, discussed in selected references from the biomedical literature. An emphasis was given to the definition of authorship stated in the available 1997 version of the "Uniform Requirements for Manuscripts Submitted to Biomedical Journals". ⋯ These and other changes should be disused to prospective authors, particularly in multicentric studies usually leading to multi-authorship. This new Editorial and a translation into Spanish of the authorship criteria contained in an-updated (2001) version of the "Uniform Requirements" are included in this issue of Revista Médica de Chile with the purpose of stimulating authors and readers to think in their implications.
