Case Reports
[Acute laryngeal dyspnea in the patient with botulism cured in 2001 year in ENT Department Hospital in Kalisz].
The authors presented symptoms of botulism, diagnostics and treatment. In this case there was a necessity of urgent tracheotomy. The authors had good results because of early treatment with serum antibotulism A+B+C and symptomatic treatment. Decannulation was only after 45 days because the paralytic symptoms declined very slowly.

The authors report the case of 68 year old male operated upon for the branchial cyst of the neck. Histopathological examination showed carcinoma in the wall of the cyst. Ipsilateral tonsillectomy was done and pathology revealed primary focus in the palatine tonsil. ⋯ Patients with carcinoma cells found in branchial cyst should be diagnosed and treated as metastasis from Waldeyer's ring to the lymph nodes of the neck. Ipsilateral tonsillectomy is indicated as blind biopsy. When histopathological examination of the tonsil is negative the patients should be treated as metastasis to the neck from unknown primary focus.

The authors present actual opinions concerning etiology, pathogenesis and diagnostics of sarcoidosis with specific evaluation of the role of T lymphocytes and cytokines in formation of sarcoid granulation. Rareness of sarcoidosis in that region and difficulties in neck tumours diagnosis is emphasizing. ⋯ It always can go into generalized form. The authors present a case of solitary sarcoidosis of the neck.

Outer hair cells are believed to be responsible for generation of otoacoustic emissions (OAEs). High sensitivity of OAE makes it possible to diagnose subclinical cochlear pathologies even if they have not yet been revealed by tonal audiometry. The aim of this study was to evaluate an influence of general anesthesia and other drugs administered in pre- and post-operative period on TEOAE in subjects operated on for various congenital heart defects in extracorporeal circulation. ⋯ Among ototoxic drugs only acetylsalicylic acid affected TEOAE in post-operative tests. The authors conclude that general anaesthesia does not change the cochlear function, although, in children in whom acetylsalicylic acid was administered in post-operative period the cochlear function was affected. In these subjects the function of the cochlea should be monitored in post-surgical period.

The usefulness of the videolaryngoscopy in patients after total laryngectomy/laryngopharyngectomy was discussed. They serve for: a) evaluation of the pharynx and of the pharyngoesophageal sphincter (pes) morphology, b) prognosis of the esophageal speech developing. In the study videolaryngoscopy was done in 82 patients (7 female and 75 male). ⋯ The shape of the pharynx in all the patients after laryngectomy with plasty or simple myotomy of the pes was regular, with thin and smooth mucosa while the rest pressure was low (3.0 +/- 1.18 kPa(-)+/- 22.5 +/- 8.8 mm Hg). In the analyzed material at the rest pressure in the sphincter area from 0.7 to 4kPa (from 5 to 30 mm Hg), 93% (41/44) of the patients have mastered the esophageal speech. It was stressed that videolaryngoscopy is entirely sufficient for the approximate assessment of the rest pressure in the pes area and prognosis of the esophageal speech development process.
