Comparison of supercapsular percutaneously assisted approach total hip versus conventional posterior approach for total hip arthroplasty: a prospective, randomized controlled trial.
Total hip arthroplasty (THA) has been one of the most successful orthopedic procedures over the past 30 years. Nowadays, the techniques of exposure for THA have undergone great changes, allowing surgeons to perform THA through mini-incisions. Recently, a novel minimally invasive surgical technique of the supercapsular percutaneously assisted total hip arthroplasty was reported in 2011. The purpose of this study was to compare the SuperPath approach with the conventional posterior approach, in terms early outcomes and radiologic results. ⋯ This prospective randomized study reveals that the SuperPath technique was associated with shorter LOS, earlier time to walk and climb, and lower postoperative pain levels. It also allowed early postoperative rehabilitation and faster recovery than conventional technique.

Previous studies have evaluated the risk factors for retear of large/massive rotator cuff tears (RCTs) that were treated arthroscopically; however, most studies did not evaluate tear patterns. The present study hypothesized that postoperative risk factors are affected by the tearing patterns in large/massive cuff tears in patients undergoing arthroscopic rotator cuff repair (ARCR). ⋯ Although multivariate analysis failed to detect significant risk factor for retear in patients with anterosuperior large/massive cuff tears who undergo ARCR, it demonstrated that active external rotation less than 25° before surgery is a significant risk factor in those with posterosuperior large/massive tears. This study may help surgeons understand the results of arthroscopic surgery in patients with large/massive tears.

Studies of elderly patients with Garden-I and Garden-II femoral neck fractures (FNFs) suggest that a preoperative posterior tilt of the femoral head of at least 20° increases the risk of fixation failure. A recently published treatment algorithm recommended hemiarthroplasty over internal fixation for elderly patients with Garden-I and Garden-II FNFs and a preoperative posterior tilt of at least 20°. However, the reliability of the method used to measure the posterior tilt has not been assessed according to recommended standards for reliability trials. ⋯ Eight orthopedic surgeons measured the posterior tilt in 50 Garden-I and Garden-II FNFs and achieved excellent inter- and intrarater reliability. However, variations in repeated measurements and variations in measurements made by different raters, as assessed by the RC and the MDC respectively, ranged from 13.9° to 14.1°. The variations in posterior tilt measurements should be taken into account when choosing the type of implant for elderly patients with Garden-I and Garden-II femoral neck fractures.
