[Evaluation of the prognosis of critically ill surgical patients by APACHE II score system].
A prospective trial was conducted in 386 critically ill surgical patients, the APACHE II score was used to quantify the severity of illness and to judge the outcome. Admission APACHE II score of these patients averaged 15.6 +/- 5.4, the mortality rates rose from 1.9% to 100% when the APACHE II score increased from 0 to 38. The score was closely correlated with the subsequent risk of hospital death for these patients (r = 0.75 P < 0.001). Critically ill surgical patients would be well served by APACHE II for quality assurance or outcome.

Clinical Trial[Management of pleural adhesion in the video-assisted thoracoscopic surgery].
This article describes the pleural adhesion managements in the video-assisted thoracoscopic surgery (VATS). We have operated the VATS successfully and developed a new technique for management of diffuse cellulose symphysis. ⋯ No death and no complications occurred. Comparing with control group, the chest tube drainage time and hospital stay are not significant difference.

From 1982 to 1992, 25 patients with subclavian steal syndrome (SSS) were admitted with 20 undergoing surgery. Etiology included atherosclerosis 56% (14/25), Takayasu's disease 36% (9/25), 14 of them were smokers. Stenosis or occlusion of the left subclavian artery were found in 14, the right in 7, and bilateral in 4. 14 cases had vertigo symptoms, 24 cases had claudication of the arm, 9 of them complained transient ischemic attack (TIA). ⋯ PTA was done for a girl with innominate severe stenosis but symptom recurred three months later. Symptoms of the upper extremity ischemia were relieved in 75% of the patients, and of the cerebrovascular ischemia in 50%. Our conclusion is that surgical therapy remains the treatment of choice in symptomatic patients.

Comparative Study[Effects of different cooling rate on the viability of cryopreservation of skin].
The three methods of stored skin by vitrification and slow cooling and vitrification+slow cooling were studied in order to improve the viability of stored skin. The results showed that the viability (oxygen consumptions and succinate dehydrogenase) of skin cryopreserved by vitrification was superior than by slow cooling and vitrification+slow cooling. The viability of homograft stored by vitrification was 61% compared with that for other two groups of 51.7% and 49.2% (P < 0.05). So the skin stored by this vitrification may be suitable for clinical use.

[Changes on hemodynamics and oxygen delivery in patients with septic shock].
Since there are still some confusions in hemodynamics and changes of oxygen delivery (DO2) in septic shock which has a high mortality, the 40 septic shock patients were observed in hemodynamics and DO2 in general ICU of Peking Union Medical College Hospital. Both surviving group and nonsurviving group had their hyperdynaminc state in hemodynamics at the beginning of septic shock. In survivors, hemodynamics were back to normal range at the 4th day of septic shock. ⋯ The changes of O2 ext have a significance in prediction of outcome of septic shock. The current therapies for septic shock have remarkable limitation. So the new treatment methods of septic shock, not only to increases DO2, should be found out for improving the survival rate further.
