Historical Article[From barber-surgeons' guild to medical association. A 400th anniversary].
The barber-surgeons' guild in Bergen was founded on 17 January 1597. For nearly 250 years this was the only "medical association" in Norway. The guild disappeared with the death of its latest master, Christian Wilhelm Wisbech (1740-1822). ⋯ Their empirical knowledge was more in touch with real life than was the medicine taught by the professors. This paper describes the barber-surgeons' education and work, including their conflicts with other craftsmen and doctors. During the 18th century the barber-surgeons' education was improved, and eventually was given full academic status.

Case Reports[Analgesic effect of ketamine in a patient with neuropathic pain].
We report the effect of a single daily dose of ketamine in a 54 year old woman with fibromyalgia and severe post-traumatic neuropathic pain. A number of different approaches for pain relief had been tried with little effect. An intramuscular test dose of 0.4 mg/kg ketamine combined with 0.05 mg/kg midazolam lead to analgesia which lasted for almost two days. ⋯ This effect is distinctly different from the short-lasting (10-30 min) analgesic effect in cases of acute nociceptive pain. The long-term depression of the intensity of chronic pain states may be due to a reversal of NMDA receptor-dependent long-term potentiation of synapses in central pain pathways. By giving ketamine as a single dose at night the mental side-effects are reduced or avoided.

Historical Article[What is anesthesia? Some reflections on the definition, measuring methods and mechanisms at the 150th anniversary].
Modern anaesthesia has developed over 150 years. In spite of this, we still do not know how to explain the state of anaesthesia exactly, how to measure anaesthesia nor what the basic mechanisms are. ⋯ Anaesthesia modulates or blocks the somatic and autonomic responses to injury, mainly by postsynaptic inhibition of impulses. Empirical dosing and clinical monitoring are still the rule, computerized and electronic systems need to be further refined.
