Case Reports
[Spontaneous post-partum inhibitor to factor VIII clotting anti- gen--a cause of life-threatening hemorrhage].
Pathologic inhibitors of blood coagulation as cause of acquired haemostatic failure are rare. We report about a 23 years old primigravida with a life-threatening haemorrhage post partum. ⋯ We reacted successfully with cyclophosphamid and cryoprecipitated factor VIII. References to diagnostic and therapy of pathologic clotting factor inhibitors are described.

In the past decade modern, perioperative short-term antibiotic-prophylaxis has been widely accepted in certain gynecological operations and in caesarean section. Many studies demonstrate the clinical effectiveness. ⋯ The infectious morbidity should be carefully evaluated at every department. The older Cephalosporins (first and second generation) should be used for prophylaxis.

There is reported on a 35 years old woman, who was delivered of her third child. Six days after the expected day of delivery labour was introduced. On the end of the second stage an amniotic fluid infusion happened. ⋯ Within four hours she received 22 units of blood and 27 units of coagulo-active substance because of severe coagulation defect. After restoration of coagulability of blood hysterectomy had to follow owing to uterine rupture which in the first instance was overlooked. Woman and child survived.
