Randomized Controlled Trial[Diprivan versus midazolam in combined anaesthesia with ketamin for minor gynecological surgery].
We tested the hypothesis that diprivan/ketamine (D/K) anesthesia would offer advantages compared to midazolam/ketamine (M/K) in patients undergoing minor gynecological surgery. After patient written consent, 60 healthy women, which were scheduled for elective termination of pregnancy were randomly allocated into two groups. Operating conditions, recovery, pain, postoperative nausea and vomiting (PONV) and patient's satisfaction to anesthesia were assessed. ⋯ Immediate recovery was faster with patients given diprivan than midazolam. Patients receiving M/K experienced more PONV in recovery room. D/K is preferable method of anesthesia for ultra-short gynecological procedure compared to M/K because of faster recovery and decreased incidence of PONV.

Case Reports[Paroxysmal tachicardia during pregnancy, electro-cardioversion, delivery].
We present a rare case of high-stage paroxysmal atrial tachycardia, clinically exhibit during pregnancy. Refers to 29-years-old pregnant woman, without preceding heart disease. With advancement of pregnancy tachycardial bout getting up daily, its severity increased gradually. ⋯ Because of new extremely acceleration of heart rate and unstable hemodynamics during the supportive therapy, is undertaking second electro-cardioversion. We applied anti-recurrent drug therapy. At 36 weeks gestation, the patient manifested resistance to applied therapy, which imposed pre-term delivery by elective caesarean section in interest of the mother and fetus, by the presence of cardiologist.

It is observed that the combined oral hormonal contraceptives affect significantly haemostasis and induce changes of many factors taking part in it. Some of them increase their concentrations, while others decrease. The changes in haemostasis during oral contraceptive use with third generation gestagens and same dose of ethynilestradiol and the biological plausibility for eventually increased risk of VTE at women using these drugs, are not cleared and continue to be an object of investigation and discussion.

Pentalogy of Cantrell is a rare congenital anomaly consisting of the following features: (1) midline supraumbilical abdominal wall defects (2) deficiency of the anterior diaphragm (3) defects in the diaphragmatic pericardium (4) defects of the lower sternum (5) congenital cardiac malformation We report a case of Cantrell's Pentalogy with rare cardiac malformation single ventricular chamber with two atria ventricular valves and pulmonary stenosis. The newbom had multiple congenital anomalies: omphalocele, thoracoabdominal ectopia cordis, diaphragmatic defect, short sternum/sternal defect and dysmorphic features--microcephaly, hypertelorism, down slanted palpebral fissures, micrognathia, low set ears.
