Clinical excellence in psychiatry: a review of the psychiatric literature.
The provision of excellent patient care is a goal that physicians would like to achieve in caring for all patients, all of the time. Until recently, clinical excellence had not been defined, and the extent to which this recently published definition applies to the care of patients with psychiatric illness is not known. This article sets out to consider how the paradigm for clinical excellence applies to the field of psychiatry. ⋯ This review of the case reports published in psychiatry reveals that the definition of clinical excellence described in this article may be highly applicable to those caring for patients with psychiatric illness.

Pediatricians' self-reported role in treating children and adolescents with major depressive disorder: a national random survey.
Major depressive disorder (MDD) is a serious US public health problem for children and adolescents. This study explored pediatricians' self-reported role in treating children and adolescents with DSM-IV-TR MDD after the 2004 US Food and Drug Administration black-box warning. ⋯ On the basis of self-report, the majority of US pediatricians do not treat children and adolescents with MDD but instead refer these patients to psychiatrists. In light of the current shortage of child and adolescent psychiatrists in the United States, referral to these specialists may be problematic.

A Review of Potential Adverse Effects of Long-Term Opioid Therapy: A Practitioner's Guide.
Review, synthesize, and summarize recent evidence on adverse effects of long-term opioid treatment for noncancer pain and present an organ system-based guide for primary care physicians in initiating and monitoring patients receiving chronic opioid therapy. ⋯ The diverse adverse effects potentially caused by chronic opioid therapy support recommendations for judicious and selective opioid prescribing for chronic noncancer pain by primary care physicians. Additional research clarifying the risks and management of potential adverse effects of chronic opioid therapy is needed to guide clinical practice.
