To monitor longitudinally the concentrations of cytokines in the plasma of patients with severe burns. ⋯ There are significant time-dependent changes in plasma concentrations of IL-1beta, IL-1ra, IFN-gamma and TNF-alpha after serious burns. IL-1ra concentrations may be influenced by size of the burn and the acute phase response; IL-1beta, IL-1ra and IL-8 may have a role in the host's response to infection; and IL-1beta may influence outcome.

To compare gastric tonometry (pHi) with estimates of pHi in ill injured patients, and to correlate pHi with haemodynamic variables. ⋯ Experienced intensive care physicians tended to overestimate visceral perfusion, which suggests that gastric tonometry adds useful information over and above routine haemodynamic indices. Arterial blood pressure and mixed venous oxygen saturation correlated better with measured pHi than with other indices of perfusion.

To investigate the impairment of pulmonary function and complications after laparoscopic compared with open cholecystectomy through an upper midline incision. ⋯ Laparoscopic cholecystectomy causes less impairment of lung function than cholecystectomy through an upper midline incision.

Percutaneous drainage of abdominal abscesses: are large-bore catheters necessary?
To find out whether small-bore catheters (7 F) are as effective as the 14F sump drains generally used for drainage of abdominal abscesses. ⋯ We conclude that percutaneous drainage with small-bore catheters is as effective as drainage with bigger tubes.
