Comparative Study
Comparative efficacy of pregabalin and therapeutic ultrasound versus therapeutic ultrasound alone on patients with post stroke shoulder pain.
The study was designed to compare the efficacy of pregabalin, an anticonvulsant on pain behavior of stroke patients with shoulder pain in a comparative study with ultrasound therapy and ultimately to recommend a better treatment option to improve pain and function in post stroke shoulder pain. This study was carried out in a private neurology Hospital in Dhaka during January to December 2010. Data were collected from 70 post stroke patients with shoulder pain and were divided into two groups. ⋯ Marked improvement in shoulder pain were observed in both groups and Group A had better improvement in pain than group B. Therapeutic ultrasound when given with pregabalin found to have added benefit over therapeutic ultrasound alone in post stroke shoulder pain. Differences were statistically significant.

Randomized Controlled Trial Comparative Study
Comparison of efficacy of labetalol and fentanyl for attenuating reflex responses to laryngoscopy and intubation.
Stress response due to laryngoscopy and intubation has been universally recognized phenomenon resulting in increase in heart rate, arterial, intracranial, and intraocular pressure. Various pharmacological approaches have been used to blunt or attenuate such pressure responses. This prospective, randomized, placebo controlled, double blinded study was designed to compare the efficacy of bolus dose of Labetalol and Fentanyl for attenuating reflex responses to laryngoscopy and intubation. ⋯ There were an increase in heart rate, systolic, diastolic, mean arterial pressures and rate pressure product in all the three groups after intubation in comparison to base line value. But the rise was minimum in L and F group as compared to C group which is statistically significant and also minimum in L group as compared to F group. So Labetalol is better agent for attenuation of laryngoscopic and intubation reflex.

Endometriosis is presence of functioning endometrial tissue outside the uterine cavity. Endometriosis can sometimes occur in a previous surgical scar. Scar endometriosis is rare and difficult to diagnose. ⋯ Medical treatment was not helpful. The patient required wide surgical excision of the lesion. Now the patient is under regular follow up, because there is chance of recurrence.

This study was conducted to determine the association between prolonged smokeless tobacco (ST) use for more than 5 years by mothers and pregnancy outcome. It was a retrospective cohort study done on department of paediatrics and department of gynecology & obstetrics, Dhaka Medical College Hospital, Dhaka from January 2009 to December 2010. A total of 340 mother-neonate pairs were included in the study. ⋯ Frequency of spontaneous abortion, stillbirth, preterm delivery and LBW baby in case & control groups was assessed. Smokeless tobacco used by mothers for >5 years had significant association with spontaneous abortion (p<0.01), stillbirth (p<0.01), preterm delivery (p<0.001) & LBW baby (p<0.001) and carries a risk of having spontaneous abortion, stillbirth, preterm delivery & LBW 2.2 times, 2.1 times, 2.9 times & 3.3 times respectively more than non users of smokeless tobacco. It can be concluded that prolonged use of smokeless tobacco for more than 5 years is significantly associated with spontaneous abortion, stillbirth, preterm delivery and LBW baby.

Low back pain (LBP) is a common problem affecting 70% of the population in the developed countries and prevalence is also increasing in the developing countries. We conducted a clinic based cross-sectional study in the city of Dhaka and Chittagong during the period of January 2009 to December 2011. Participants (n=357) were interviewed and examined through a structured questionnaire after a non purposive sampling to explore the influence of body mass index (BMI) on LBP. ⋯ Statistically the association of LBP with BMI was found to be highly significant (p<0.0001). In this study, 137(46.78%) participants of normal BMI presented with LBP but majorities (n=82) presented with mild LBP and overweight and obese participants mostly presented with severe LBP. On the basis of these findings, careful evaluation of risk factors and deliberate assessment of the therapeutic as well as preventive measure for the management of patient of LBP is highly recommended.
