There is no consensus on whether restarting TNF inhibitors (TNFis) after treatment of an active tuberculosis (TB) infection caused by previous TNFi exposure is safe. In this study we sought to determine the safety of resuming TNFis in patients following TB treatment. ⋯ Our results suggest that resuming TNFi therapy in patients following adequate TB treatment is safe, even before completion of TB treatment.

Central neural mechanisms of interindividual difference in discomfort during sensorimotor incongruence in healthy volunteers: an experimental study.
It has been reported that disturbance in sensory and motor function may induce sensorimotor incongruence and produce pain, discomfort and other sensations in healthy volunteers. One study suggested that sensorimotor incongruent information to healthy subjects results in increased neuronal activity in the posterior parietal cortex (PPC) and dorsolateral prefrontal cortex; however, this study did not take into consideration the discomfort induced by sensorimotor incongruence. The present study attempted to characterize intracortical electrical activities for sensorimotor incongruence in the frequency domain. In our study, electroencephalogram (EEG) cortical sources were compared between sensorimotor congruence and sensorimotor incongruence. In addition, high and no discomfort subgroups were compared during sensorimotor incongruence. ⋯ The present findings suggest that the ACC and PCC are more activated in the high-discomfort subgroup than in the no-discomfort subgroup during sensorimotor incongruence. This method may evaluate the effectiveness of new medication therapy and/or rehabilitation by assessing the difference in the neuronal activity of chronic patients before and after treatment.

The aim of this study was to determine demographic, clinical, articular and laboratory variables predicting etanercept response in JIA. ⋯ Parameters predicting a high-grade response to a 6-month course of etanercept were identifiable and included age at treatment start, disease duration before treatment, JIA category other than systemic arthritis, CHAQ, parents' global assessment and concomitant corticosteroids.

Complex regional pain syndrome (CRPS) describes a broad spectrum of symptoms that predominantly localize to the extremities. Although limb fracture is one of the most frequently reported triggering events, few large-scale studies have shown the occurrence of and factors associated with CRPS following limb fracture. This study aimed to show the occurrence and identify of those factors. ⋯ Although a limited number of CRPS patients were analysed in this study, reduced operative time might help to prevent the development of acute CRPS following limb fracture.

International periodic fever, aphthous stomatitis, pharyngitis, cervical adenitis syndrome cohort: description of distinct phenotypes in 301 patients.
The aims of this study were to describe the clinical features of periodic fever, aphthous stomatitis, pharyngitis and cervical adenitis (PFAPA) and identify distinct phenotypes in a large cohort of patients from different countries. ⋯ We describe the largest cohort of PFAPA patients presented so far. We confirm that PFAPA may present with varied clinical manifestations and we show the limitations of the commonly used diagnostic criteria. Based on detailed analysis of this cohort, a consensus definition of PFAPA with better-defined criteria should be proposed.
