Case ReportsCalciphylaxis in a renal transplant recipient with normal graft function: A case report and review of literature.
Calciphylaxis, also known as calcific uremic arteriolopathy, is a rare, life-threatening complication of end-stage renal disease. However, it may also occur in patients without renal failure, the nonuremic calciphylaxis. Most patients present with painful skin ulcers. ⋯ Aberrations in calcium, phosphate, and parathyroid hormone physiology are common and biopsy is diagnostic. Early diagnosis and correction of vascular and metabolic aberrations may assist in healing as may the use of sodium thiosulfate and hyperbaric oxygen therapy. We report on successful treatment of a case of calciphylaxis in a renal transplant recipient with normal allograft function.

Assessment of knowledge and attitude toward organ donation among the Saudi population in Riyadh City.
We aimed to assess the knowledge and attitude of the Saudi population and students of health science colleges as well as healthcare workers in Riyadh city about organ donation (OD). Data of this study were collected from 648 Saudi residents of Riyadh city using interviewer-administered questionnaires and online surveys. The participants included 503 from the general population and 145 health science professionals. ⋯ The majority of Riyadh's population knew the concept and procedure of OD; however, there was a lack of awareness of the presence of SCOT. Religion is not a barrier for OD in the Saudi population. Awareness programs are urgently needed to emphasize the importance of donation as well as clarification of wrong beliefs.

Hypervolemia and high tumor necrosis factor-α: An interesting finding during the first six months' posttransplant period in kidney transplant recipients with good renal function.
Renal transplantation is the treatment of choice for the end-stage renal disease. Hypervolemia and inflammation are commonly overlooked. We investigated whether hyper-volemia develops in kidney transplant recipients with estimated glomerular filtration rate (eGFR) >70 mL/min and if there is there any correlation between inflammatory cytokines and hypervolemia in the 1st month and at six months after transplantation. ⋯ There was positive correlation between volume overload and serum TNF-α levels in KTRs. Compared to baseline, parameters including volume overload, BMI, and FTI were found to be improved at six months' posttransplant in KTRs. Interestingly, volume overload and high levels of serum TNF-α continued at least six months after transplantation.

Comparative Study Observational StudyScoring systems and outcome of chronic kidney disease patients admitted in intensive care units.
The outcome of chronic kidney disease (CKD) patients admitted to the Intensive Care Unit (ICU) is difficult to predict. This study assessed the outcome of CKD patients admitted to the ICU and evaluated prediction of 30-day mortality using the Acute Physiology and Chronic Health Evaluation (APACHE II), Simplified Acute Physiology Score (SAPS II), and Sequential Organ Failure Assessment (SOFA) score. One hundred consecutive CKD patients admitted to the ICU at a tertiary care hospital, Ahmedabad between 2011 and 2013 were included prospectively. ⋯ About 85% of patients were on intermittent hemodialysis and 15% of patients were on continuous venovenous hemodiafiltration. Sepsis was the main reason for hospital admission, and the mean length of stay in the ICU was 7.74 ± 5.34 days. The study indicates that all three scores (APACHE II, SAPS II, and SOFA) perform equally well and have equal diagnostic utility in predicting 30-day mortality.

Observational StudyAssessment of prevalence and clinical outcome of frailty in an elderly predialysis cohort using simple tools.
The relationship between frailty and chronic kidney disease in elderly population has been recognized; however, studies concentrating on frailty in predialysis patients are limited. For nephrologists, the recognition of frailty is important as it has impact on decisions on the choice of dialysis modality and sometimes on whether dialysis is indeed in the patients' best interests. Many of the tools for routine assessment of frailty are not easily applicable to those clinicians not practicing elderly care medicine. ⋯ Our study shows that the prevalence of frailty and mortality rate is high among elderly predialysis patients. Frailty and Charlson comorbidity index are independent predictors of outcome in this population. PD might be a better option of dialysis modality compared to HD in the frail population.
