Laryngoscopy and intubation incur hemodynamic changes like increase in heart rate, arterial blood pressure, pulmonary artery pressure, wedge capillary pressure and arrhythmias. Anesthesiologists are continually in search of ways to alleviate such complications. Several medicinal methods have been suggested that serve the purpose including the administration of intravenous magnesium sulfate to minimize these unfavorable responses. This study compares the effects of intravenous administration of lidocaine and magnesium sulfate on unwanted hemodynamic responses following laryngoscopy and intubation in elective surgery candidates. ⋯ Magnesium sulfate is more effective than lidocaine in controlling hemodynamics, although it may increase the heart rate.

Pulmonary alveolar proteinosis (PAP) is rare. It is characterized by the accumulation of proteinaceous materials in the alveoli. Typical appearance of BAL fluid (BALF) and positive PAS staining of BALF in conjunction with typical clinical and radiographic manifestations may be diagnostic of PAP. ⋯ It provides long-lasting benefits. We report here our experiences with segmental lung lavage by FOB in a patient with vary severe PAP since she could not undergo whole long lavage under general anesthesia. The one year follow up results are also reported.

Arterial blood gas (ABG) analysis is useful in evaluation of the clinical condition of critically ill patients; however, arterial puncture or insertion of an arterial catheter may cause many complications. The aim of this study was to determine whether venous blood gas (VBG) values can be used as an alternative to arterial blood gas measurements in patients under mechanical ventilation. ⋯ Venous blood gases, especially pH, Base excess, and PCO2 levels have relatively good correlation with ABG values. Because this correlation is not close, VBG cannot substitute ABG in mechanically ventilated patients.

At present, air way support plays pivotal role in management of patients in the ICU (Intensive Care Unit) and also RCU (Respiratory Care Unit). Ventilator weaning is an important step in the care of ICU and RCU patients. It is the gradual removal of mechanical ventilatory support. Different predictors are used for initiation of weaning. This study was designed to investigate the rapid shallow breathing index (RSBI) as a predictor for successful weaning. ⋯ Although RSBI <105 is a helpful index for weaning, application of RSBI alone may mislead the physicians. General status of the patient, concomitant diseases and duration of hospital stay should all be considered for successful weaning.

A 15- year-old boy was referred to Imam Reza Hospital with a right chest tube and chylothorax for 40 days. The patient had respiratory distress and undergone refractory treatment for chylothorax. The fluid content was chyle-rich in lipids. ⋯ One year follow up of the patient showed excellent result. In our knowledge, thoracic duct cyst occurring as a result of a delayed chylothorax and chyloperitoneum has not been reported in the literature. Surgical thoracic duct ligation can be the treatment of choice.
