[Diagnostics of Interstitial Lung Diseases - Practical Instructions with a Focus on Rheumatologic Systemic Diseases].
Interstitial lung diseases (ILD) are etiologically heterogeneous with unknown and known causes like rheumatologic systemic diseases differing in their therapeutic and prognostic consequences. In consensus between pulmonologists, rheumatologists, radiologists, and pathologists, we developed practical instructions for ILD diagnosis in rheumatologic systemic diseases, in particular because ILD can present in early stages of rheumatic systemic diseases. ⋯ The occurrence of acute exacerbations is especially critical. They are often the causes for ILD progression and are associated with considerable mortality.

Review[Drug therapy interventions for acute SARS-CoV-2 infection: who, when, and what?]
There is a variety of drug therapy options for treatment of acute SARS-CoV-2 infections. The updated S3 guideline "Recommendations for inpatient therapy of patients with COVID-19" provides clear recommendations in this regard. Which therapy is best suited for which patient and in which phase of the disease must be decided individually based on risk factors, comorbidities and contraindications. This article provides an overview.

The prevalence of post-COVID syndrome (PCS) has not yet been conclusively clarified. The existing definitions primarily reflect temporal aspects, but disregard functional deficits as well as the objectification of symptoms. ⋯ The underlying definitions as well as temporal inclusion criteria were extracted. 16 systematic reviews were included, 11 of which included a definition of PCS. In 58 % of the individual studies analyzed, patients with symptomatology > 12 weeks and thus according to the definition of PCS were included. Further clarification of the definition of PCS is necessary to facilitate diagnosis and multimodal treatment and to use the scarce therapeutic resources accordingly.

Case Reports[Case report about an initial diagnosis of HIV-infection in a young sailor through two AIDS-defining diseases with lethal progression].
We saw a previously healthy 30-year-old Southeast-Asian sailor with progredient coughing and fever. ⋯ This case demonstrates the difficulties in the pharmacotherapy and with drug interactions in a HIV late presenter with multi organ failure. Consultation and advice of the hospital pharmacy and the antibiotic stewardship team was vital for treating this patient. In the end cases like the portrayed should be treated in specialized clinics.

[When COVID does not heal: post-COVID courses with fatigue and exercise intolerance].
Post-COVID-Syndrome consists of several pathobiologically different entities, ranging from damage to specific organs to new onset autoimmune diseases. This article focuses on post-COVID courses with the leading symptoms of fatigue and exercise intolerance. This category is also known from other infectious diseases and includes myalgic encephalomyelitis/chronic fatigue syndrome.
