Combined transcervical and unilateral-thoracoscopic thymectomy for myasthenia gravis: 2 years of follow-up.
To investigate the efficacy of combined transcervical and unilateral-thoracoscopic thymectomy for myasthenia gravis. ⋯ Thymectomy represents a safe and valid approach for patients with myasthenia gravis. Achieving a curative thymectomy and good cosmesis in myasthenic patients is possible with the combined transcervical and unilateral-thoracoscopic thymectomy as an effective alternative to open approaches.

Case ReportsSuccessful treatment of parastomal varices bleeding with percutaneous N-butyl-2-cyanoacrylate glue injection.
Parastomal varices are known complication of stoma creation in patients with portal hypertension. Similar to esophageal and gastric varices, bleeding can occur and can lead to hepatic decompensation and even death. Diagnosis of parastomal bleeding may be delayed if not suspected. ⋯ We report a case of a patient with carcinoma of the cervix that was complicated by a rectal stricture postradiotherapy requiring a diversion colostomy. She also had end stage chronic liver disease secondary to chronic hepatitis C infection. She presented with 3 episodes of parastomal varices bleeding that was not initially suspected and was successfully treated with 1 session of percutaneous N-butyl-2-cyanoacrylate (Histoacryl) glue injection.

Effects of pneumoperitoneum and positioning on respiratory mechanics in chronic obstructive pulmonary disease patients during Nissen fundoplication.
The aim of this study was to evaluate the effects of pneumoperitoneum and patient positioning on respiratory mechanics and blood gases in chronic obstructive pulmonary disease (COPD) patients during laparoscopic Nissen fundoplication. ⋯ In COPD patients, Trendelenburg position has negative effect on respiratory mechanics. Pneumoperitoneum and other positions have similar effect on respiratory mechanics in COPD and control patients.

Case ReportsSurgical excision of large multinodular goiter using an endoscopic transaxillary approach: a case report.
Minimal access surgery for thyroid and parathyroid disease has gained increasing popularity due to excellent endoscopic visualization and overall cosmetic outcome. Most current techniques limit the size of the gland that can be removed to less than 4 cm. Patients with multinodular goiter with gland size greater than 4 cm commonly present for surgical therapy. We evaluated the use of an endoscopic transaxillary approach for the treatment of large multinodular goiters. We herein present a case report of 3 consecutive patients undergoing this technique for benign multinodular goiter disease. ⋯ Transaxillary endoscopic thyroidectomy may be a viable option to open cervical thyroidectomy in the treatment of patients with large multinodular goiter. Laparoscopic and open inguinal hernia repair with alloplastic material: do the subjective and objective parameters differ in the long-term course?
Management with alloplastic materials has become the standard procedure in inguinal hernia surgery. The aim of this study was to evaluate the long-term results after laparoscopic and open groin hernia repair with alloplastic material in a large patient population. ⋯ Whereas the Lichtenstein procedure was not significantly better in any of the evaluated parameters, TEP was superior especially in the subjective parameters.
