ReviewMicrobiology of genus Raoultella, clinical features and difficulties in its diagnosis
The genus Raoultella, which belongs to the Enterobacteriaceae family, encompasses gramnegative, oxidase-negative, aerobic, encapsulated and motionless bacilli. Currently, four species of this genus have been described: Raoultella terrigena (R. terrigena), Raoultella planticola (R. planticola), Raoultella ornithinolytica (R. ornithinolytica) and Raoultella electrica (R. electrica), out of which R. planticola and R. ornithinolytica are the most important because of their probable association as pathogenic agents. ⋯ The rapid and correct detection has been increasing since the advent of mass spectrometers (MALDI-TOF MS), which have been used for the differentiation of Klebsiella spp. and Raoultella spp. Currently 80 cases of bacteremia secondary to Raoultella spp. have been identified with different sites of primary infection, which is why it is important to emphasize proper detection in hospitals and healthcare facilities.

Extracorporeal membrane oxygenation as a bridge for heart transplantation by Chagas cardiomyopathy
Extracorporeal membrane oxygenation is a technique indicated in cases of severe respiratory failure or in situations where pump failure or heart failure is refractory to conventional medical treatment. Our goal was to describe the results of a patient with chronic Chagas dilated cardiomyopathy treated with extracorporeal membrane oxygenation as bridging therapy for heart transplantation. ⋯ Extracorporeal membrane oxygenation is a temporary, viable, safe and effective alternative for patients with severe heart failure refractory to treatment who will undergo transplantation.

Case Reports[Anesthesic management during total organic separation of pygopagus conjoined twins].
Conjoined twins are among the most rare and challenging congenital malformations. The incidence varies from 1:30 000 to 1:200 000 live births. Pygopagus twins account for about 10 to 18% of all conjoined twins. ⋯ Each type of siamese has specific characteristics depending on the type of union; comprehensive knowledge of the morphology and physiopathology of the twins is essential for optimal anesthetic management.

Sleep disturbances are associated with frailty in older adults
Sleep plays a vital role in good health. Since sleep disturbances have been linked to a series of adverse physical health outcomes, the objective was to analyze the association between sleep disturbances and the frailty criteria in Mexican older adults from Mexico City. ⋯ Sleeping more than usual or having trouble sleeping contribute to increase frailty in older people from Mexico City.

[Level of teaching competence at the Undergraduate Medical Internship of UNAM's Faculty of Medicine].
There is no systematic evaluation of teaching performance in the clinical area at UNAM Faculty of Medicine. The study purpose is to assess the teaching competence level in the Undergraduate Medical Internship (UMI). ⋯ The teachers of the UMI are considered educators with high level of teaching competence, according to the perceptions of the undergraduate internal doctors. The evaluation of teaching competence level is very important for institutions that look for the continuous professional development of its faculty.
