Case ReportsSevere Hypernatremia Caused by Acute Exogenous Salt Intake Combined with Primary Hypothyroidism.
This report describes a case of severe hypernatremia with a serum sodium concentration of 188.1mmol/L caused by exogenous salt intake. A 26-year-old man diagnosed with Crohn's disease 5 years previously visited our clinic due to generalized edema and personality changes, with aggressive behavior. He had compulsively consumed salts, ingesting approximately 154 g of salt over the last 4 days. ⋯ Primary hypothyroidism was also diagnosed. He was discharged after 9 days from admission, with a stable serum sodium level. We have described the possibility of successful treatment in a patient with hypernatremia caused by acute salt intoxication without sustained hypotonic fluid therapy.

Disturbances in fluid and electrolytes are among the most common clinical problems encountered in the intensive care unit (ICU). Recent studies have reported that fluid and electrolyte imbalances are associated with increased morbidity and mortality among critically ill patients. To provide optimal care, health care providers should be familiar with the principles and practice of fluid and electrolyte physiology and pathophysiology. ⋯ Water deficit should be corrected with hypotonic fluid, and ongoing water loss should be taken into account. Cardiac manifestations should be identified and treated before initiating stepwise diagnostic evaluation of dyskalemias. Divalent ion deficiencies such as hypocalcemia, hypomagnesemia and hypophosphatemia should be identified and corrected, since they are associated with increased adverse events among critically ill patients.

Uncoventional views on certain aspects of toxin-induced metabolic acidosis.
This discussion will highlight the following 9 specific points that related to metabolic acidosis caused by various toxins. The current recommendation suggests that alcohol dehydrogenase inhibitor fomepizole is preferred to ethanol in treatment of methanol and ethylene glycol poisoning, but analysis of the enzyme kinetics indicates that ethanol is a better alternative. In the presence of a modest increase in serum osmolal gap (<30 mOsm/L), the starting dose of ethanol should be far less than the usual recommended dose. ⋯ Hemodialysis is important in the treatment of methanol poisoning and ethylene glycol poisoning with renal impairment, with or without fomepizole or ethanol treatment. Severe leucocytosis is a highly sensitive indicator of ethylene glycol poisoning. Uncoupling of oxidative phosphorylation by salicylate can explain most of the manifestations of salicylate poisoning.

Clinical Significance of Strong Ion Gap: between ICU and Hemodialysis Patients with Metabolic Acidosis.
Metabolic acidosis is the most frequent acid-base disorder in critically ill patients and dialysis patients. This study is to compare the conventional approach with the physicochemical approach between the intensive care unit (ICU) and hemodialysis (HD) patients. Fifty-seven ICU patients and 33 HD patients were enrolled. ⋯ Concerning the contributions of the three main causes of metabolic acidosis, increased SIG was comparable between the ICU and HD group (n=48, 90.6% vs n=30, 93.8%), whereas hyperlactatemia (n=9, 17.0% vs n=0, 0%) and hyperchloremia (n=20, 35.1% vs n=2, 6.1%) was significantly increased in the ICU group compared with the HD group. Multiple underlying mechanisms are present in most of the ICU patients with metabolic acidosis compared with the HD patients. In conclusion, the physicochemical approach can elucidate the detailed mechanisms of metabolic acidosis in ICU and HD patients compared with conventional measures.

D-Lactic acidosis has been well documented in ruminants. In humans, D-lactic acidosis is very rare, but D-lactic acidosis may be more common than generally believed and should be looked for in a case of metabolic acidosis in which the cause of acidosis is not apparent. ⋯ The entity should be considered as a diagnosis in a patient who presents with metabolic acidosis accompanied by high anion gap, normal lactate level, negative Acetest, history of short bowel syndrome or malabsorption, and characteristic neurologic manifestations. Low carbohydrate diet, bicarbonate treatment, rehydration, and oral antibiotics would be helpful in controlling symptoms.
