Case ReportsAcetabular reconstruction with a custom-made triflange acetabular component through direct anterior approach - A case report.
Complex acetabular reconstruction sometimes requires the use of a custom-made triflange acetabular component (CTAC). In this article we describe the surgical technique to achieve its implantation through the direct anterior approach (DAA). Meanwhile we report on our first aMace® CTAC placement in revision total hip arthroplasty through the DAA. ⋯ Because of the documented lower dislocation rate and a faster early rehabilitation of procedures performed through DAA, we considered to implant this vast acetabular reconstruction component through the same approach we use for our primary and standard revision cases. In collaboration with the engineers, the design and the screw direction were adapted to the DAA. We used a standard DAA with longitudinal incision and had no difficulty to successfully implant the CTAC in the desired implant position.

A three-year retrospective multi-center study on time to surgery and mortality for isolated geriatric hip fractures.
There are multiple reports on the effect of time to surgery for geriatric hip fractures; it remains unclear if earlier intervention is associated with improved mortality, hospital length of stay (HLOS), or cost. ⋯ Level III.

Severe metallosis-related osteolysis as a cause of failure after total knee replacement.
Metallosis is a syndrome of metal-induced synovitis caused by friction between two metal surfaces. In contrast to the hip joint after resurfacing arthroplasty or metal-on-metal (MoM) total hip replacement, metallosis of the knee is extremely rare. ⋯ Metallosis-associated osteolysis should be suspected in cases with radiologically evident polyethylene wear after knee replacement. Recognizing that revision arthroplasty is very technically demanding in such cases, surgeons should have a back-up with modular revision components and a ready access to reconstructive options at this revision setting.
