Spinal cord stimulation is commonly used to treat medically intractable pain. Different stimulation designs are used to obtain pain suppression such as tonic stimulation, high frequency stimulation, and burst stimulation. Preliminary analysis of the same data used in this study demonstrated that burst stimulation likely modulates the medial pain pathways in contrast to tonic stimulation. The question arises what different and common supraspinal mechanisms burst and tonic stimulation use. ⋯ These data suggest that burst and tonic stimulation both modulate the descending pain inhibitory system (via pgACC), as well as a self-referential contextual (via PCC) aversive memory system (via parahippocampus). However, burst normalizes the pain supporting/suppressing balance in contrast to tonic mode by a greater effect on the dACC.

ReviewResponse assessment after stereotactic body radiotherapy for spinal metastasis: a report from the SPIne response assessment in Neuro-Oncology (SPINO) group.
The SPine response assessment In Neuro-Oncology (SPINO) group is a committee of the Response Assessment in Neuro-Oncology working group and comprises a panel of international experts in spine stereotactic body radiotherapy (SBRT). Here, we present the group's first report on the challenges in standardising imaging-based assessment of local control and pain for spinal metastases. ⋯ We summarise the results of an international survey of the panel to establish the range of current practices in assessing tumour response to spine SBRT. The ultimate goal of the SPINO group is to report consensus criteria for tumour imaging, clinical assessment, and symptom-based response criteria to help standardise future clinical trials.

To assess the clinical effect and safety of direct current (DC) pulse produced by Han's Acupoint Nerve Stimulator in reduction (HANS) of labor pain. ⋯ The DC pulse produced by HANS may be a non-pharmacological alternative to labor pain with fewer side effects.

The logic behind diagnostic blocks is very straightforward. What has not been determined is the way diagnostic blocks can be best used. ⋯ "The Best Likelihood Scenario" might improve diagnostic accuracy while decreasing societal costs.
