The U.S. health care sector produces approximately 10% of national greenhouse gas emissions, paradoxically harming human health. Neurosurgery is a resource-intensive specialty that likely contributes significantly, yet literature assessing this impact is absent. We investigate the difference in carbon emissions between spinal versus general anesthesia in lumbar spine surgery. ⋯ This is one of the first known comparative carbon footprint studies performed in neurosurgical literature. We highlight the dramatic carbon footprint reduction associated with using spinal anesthesia and reflect a single neurosurgeon's change in practice from using only general anesthesia to incorporating the use of spinal anesthesia. Within general anesthesia patients, desflurane use was particularly harmful to the environment. We hope that our study will pave the way toward future research aimed at uncovering and reducing neurosurgery's environmental impact.

ReviewPower spectrum and spectrogram of EEG analysis during general anesthesia: Python-based computer programming analysis.
The commonly used principle for measuring the depth of anesthesia involves changes in the frequency components of the electroencephalogram (EEG) under general anesthesia. Therefore, it is essential to construct an effective spectrum and spectrogram to analyze the relationship between the depth of anesthesia and the EEG frequency during general anesthesia. This paper reviews the computer programming techniques for analyzing the spectrum and spectrogram derived from a single-channel EEG recorded during general anesthesia. ⋯ Finally, the multitaper method, which can suppress artifacts caused by the edges of the analysis segments, suppress noise, and probabilistically infer values that are close to the real power spectral density, is explained using practical examples of the analysis. All analyses were performed and all graphs plotted using Python under Jupyter Notebook. The analyses demonstrated the effectiveness of Python-based programming under the integrated development environment Jupyter Notebook for constructing an effective spectrum and spectrogram for analyzing the relationship between the depth of anesthesia and EEG frequency analysis in general anesthesia.

Pro-Con Debate: Monitored Anesthesia Care Versus General Endotracheal Anesthesia for Endoscopic Retrograde Cholangiopancreatography.
Over the past several decades, anesthesia has experienced a significant growth in nonoperating room anesthesia. Gastrointestinal suites represent the largest volume location for off-site anesthesia procedures, which include complex endoscopy procedures like endoscopic retrograde cholangiopancreatography (ERCP). These challenging patients and procedures necessitate a shared airway and are typically performed in the prone or semiprone position on a dedicated procedural table. ⋯ The consensus opinion of the authors is that anesthesia for ERCP should be provided by a qualified anesthesia professional who weighs the risks and benefits of each technique for a given patient and clinical circumstance. This Pro-Con article highlights the many challenges anesthesia professionals face during ERCPs and encourages thoughtful, individualized anesthetic plans over knee-jerk decisions. Both sides agree that an anesthetic technique administered by a qualified anesthesia professional is favored over an endoscopist-directed sedation approach.
