Several previous trials have attempted to compare the efficacy of femoral nerve block (FNB) and local infiltrative analgesia (LIA) for patients received anterior cruciate ligament (ACL) reconstruction, but reached inconsistent conclusions. The primary purpose of this present research was to compare the FNB and LIA in the reconstruction of ACL. ⋯ It is assumed that the efficacy of LIA in the early postoperative pain is no less than that of FNB. For our study, the major limitation is the lack of randomization. Nevertheless, these data were prospectively harvested, with high response rate of patient.

Perioperative pain management in patients undergoing major surgery with restrictive lung disease can be a challenge. Facioscapulohumeral muscular dystrophy, a rare genetic disorder, causes progressive proximal weakness resulting in chronic pain. ⋯ We used interscalene brachial plexus and erector spinae plane block with catheter insertion using continuous local anesthetic infusion. The aim was to reduce potential respiratory complications in a patient with severe restrictive lung disease and reduce hospital stay.

Sacral Erector Spinae Plane Block Provides Surgical Anesthesia in Ambulatory Anorectal Surgery: Two Case Reports.
Erector spinae plane block (ESPB) is a new and popular interfacial fascial plane block which has been used in many different surgeries. There are a few cases in which ultrasound-guided sacral ESPB was used for postoperative analgesia. This article presents the successful use of bi-level, bilateral sacral ESPB for main anesthetic method in anorectal surgery. Anesthetic level required for surgery was accomplished in 30 minutes, and none of the patients experienced pain throughout the surgery. ⋯ The patients, who were contacted later, indicated no need for any analgesic for 24 h postoperatively. To the best of our knowledge, this is the first case report in the literature where sacral ESPB is used as the sole anesthetic technique. The sacral ESPB can be considered in anorectal surgery as an alternative technique for spinal or general anesthesia.

The analgesic efficacy of ultrasound-guided transversus abdominis plane (TAP) block combined with oral multimodal analgesia in comparison with oral multimodal analgesia after caesarean delivery: a randomized controlled trial.
The transversus abdominis plane (TAP) block is used increasingly in parturients after caesarean delivery. This is a randomized controlled trial to evaluate the effectiveness of bilateral single-shot of TAP blocks in patients who received multimodal oral analgesia for postoperative pain relief. ⋯ Bilateral single-shot of TAP blocks confer little additional benefit when a multimodal oral analgesic regimen is used for pain control after caesarean section under spinal anaesthesia.

Quadriceps Weakness After Single-Shot Adductor Canal Block: A Multivariate Analysis of 1,083 Primary Total Knee Arthroplasties.
Adductor canal blocks (ACBs) are commonly employed in multimodal pain control for total knee arthroplasty (TKA) and minimize motor blockade compared with femoral nerve blocks. Quadriceps weakness may be associated with ACBs. The purpose of this study was to quantify the prevalence of clinically relevant quadriceps weakness after a single-shot ACB and to identify the factors that are associated with its diagnosis. ⋯ Therapeutic Level IV.
