Retrospective analysis of assisted conception techniques: A three year experience.
In this retrospective survey, we present the results of infertility treatment by various assisted conception techniques at the assisted conception unit, New Jeddah Clinic Hospital, Jeddah, Saudi Arabia. During a three year period from January 1990 to December 1992, a total of 249 treatment cycles were started in patients with long-standing infertility. There were 107 ovulation induction cycles carried out in patients with chronic anovulation and patent tubes and normal spermogram using rigorous ovulation induction therapy with intensive ultrasound monitoring. ⋯ A total of 54 pregnancies resulted from these treatments, giving an overall pregnancy rate of 21.6%. The highest pregnancy rate of 32.35% is seen in the CSO and intrauterine insemination (IUI) in the GIFT. The pregnancy rate in the IVF/ET group was 22.2%/oocyte pickup and 30.8%/embryo transfer.

Randomized Controlled Trial Clinical Trial[Inguinal funicular block in vasectomy].
The analgesic efficacy of inguinal funicular block with 10 ml carbocaine 1% as a supplement to local infiltration analgesia of the vas deferens was investigated in patients undergoing vasectomy. Pain/discomfort during vasectomy and on the first and third day postoperatively were investigated using a questionnaire. ⋯ There was significantly less intraoperative pain on the side of the active inguinal funicular block (p < 0.0001), but no significant differences were found at the first and third postoperative day (p = 1.16-1.19). Inguinal funicular block can be recommended as a supplement to the usual use of local infiltration analgesia of the vas deferens.

The number of AIDS patients is steadily increasing. According to the literature these patients are often in severe pain. ⋯ The high incidence of complicated neuropathic pain syndromes in AIDS patients requires a sophisticated therapeutic approach. Closer cooperation between AIDS specialists and pain specialists, comparable to that already existing for other patient groups, is therefore desirable.

Nociceptive stimuli are modulated at the dorsal horn of the spinal cord. This modulation is performed by various systems working independently complementarily, additively or supra-additively. Non-opioid analgesics relieve pain without a motor blockade. ⋯ Lysine acetylsalicylic acid (L-ASA) has been given intrathecally for the therapy of severe cancer pain and chronic back pain. In most patients good analgesia was observed up to 2 months after a single injection. If neurotoxity can be excluded, L-ASA may be an alternative in the therapy of cancer pain before neurodestructive therapy is done.

A multidisciplinary approach, acknowledged as being the best treatment strategy for a wide range of chronic pain patients, requires cooperation at least between trained professionals in the medical and the psychological disciplines. Psychological criteria are formulated to bridge persisting gaps in the interdisciplinary implementation of pain treatment strategies.
