To investigate the characteristics of elderly-onset primary Sjögren syndrome (pSS) using chest high-resolution computed tomography and pulmonary function tests (PFTs). The data of 102 patients with pSS with interstitial lung disease were retrospectively analyzed. The chest high-resolution computed tomography, PFTs, and clinical and laboratory data were evaluated based on the age of onset: elderly-onset pSS (EopSS) (≥65 years) versus adult-onset pSS (AopSS) (<65 years). ⋯ Usual interstitial pneumonia is more common in the EopSS group. Although there is no significant difference in ventilation dysfunction between the EopSS and AopSS groups, small airway disease is more common in the EopSS group, while restrictive ventilatory dysfunction is more common in the AopSS group. Therefore, the EopSS group has its own characteristics and it is worth studying and noting.

In South Korea Long-Term Care Insurance (LTCI) system, the special dementia rating (SDR) is a registration grading for dementia patients who do not have a physical disability or functional restrictions and is the first applicable registration following the diagnosis of dementia. We investigated the differences in age of registration of SDR and age of dementia diagnosis according to the educational level and residential area. This was a retrospective, cross-sectional study using the Korean National Health Insurance Service dataset. ⋯ Urban residents were diagnosed with dementia at a significantly lower age and registered for SDR earlier than rural residents (P < .001 for both). Both urban and rural residents consistently showed that a higher educational level was associated with lower age at the dementia diagnosis and SDR registration. Patients who were highly educated and living in urban areas were diagnosed with dementia and registered on SDR when they were relatively younger, indicating that cognitive decline sensitivity and medical accessibility are related to earlier dementia diagnosis and registration.

Clinically, it has been found that the Yuji point (LU10) can relieve anxiety by regulating Qi. ⋯ Acupuncture at LU10s can regulate anxiety by upregulating or downregulating the relevant brain regions and networks. LU10s can be used to treat not only lung disorders but also related mental disorders.

Impact of Adrenalectomy on Diastolic Cardiac Dysfunction in Patients with Primary Aldosteronism.
Poor prognostic cardiac function is known among some patients with primary aldosteronism (PA). However, studies with echocardiograms on whether the normalization of aldosterone after laparoscopic adrenalectomy (LADX) improves myocardial hypertrophy and diastolic cardiac dysfunction have been inadequate. Between August 2009 and December 2021, 147 patients with unilateral PA who underwent pre- and post-LADX echocardiography at a single center were enrolled in this retrospective study. ⋯ Furthermore, laparoscopic adrenalectomy improved the low and high echocardiographic values of E/A and LVEF, respectively, in both groups. The risk factors for not reaching complete CS were male sex (OR 3.42), low preoperative E/A (OR 3.11), and postoperative supranormal LVEF (OR 3.17). Although low preoperative E/A and postoperative supranormal LVEF are associated with poor clinical outcomes, LADX can improve diastolic cardiac function in patients with PA.

Sodium bicarbonate for kidney transplant recipients with metabolic acidosis in Switzerland: a multicentre, randomised, single-blind, placebo-controlled, phase 3 trial.
Metabolic acidosis is common in kidney transplant recipients and is associated with declining graft function. Sodium bicarbonate treatment effectively corrects metabolic acidosis, but no prospective studies have examined its effect on graft function. Therefore, we aimed to test whether sodium bicarbonate treatment would preserve graft function and slow the progression of estimated glomerular filtration rate (GFR) decline in kidney transplant recipients. ⋯ Swiss National Science Foundation.
