Objectives: In the context of COVID-19 pandemic, the aim of this manuscript is to provide a standard of care of patients with ARDS for non-emergency medicine trained physicians who are not customary with mechanical ventilation. Methods: We conducted a systematic review of the literature to investigate the best practice recommendations regarding the mechanical ventilation of patients with ARDS. ⋯ This focus is particularly addressed to physicians who are not experienced in the invasive respiratory management of ARDS patients. Nevertheless, it remains fundamental to acknowledge that new insights concerning this quickly spreading illness become available on a regular base.

Clinical characterization of hospitalized COVID-19 patients during the second wave of pandemic in the district of Rohrbach, Upper Austria : A single center retrospective study.
During the peak of the second wave of the coronavirus disease 2019 (COVID-19) pandemic in November 2020, the district of Rohrbach, Upper Austria, was reported to have had the highest 7‑day incidence of severe acute respiratory syndrome coronavirus‑2 (SARS-CoV-2) positive cases worldwide. In this study, we present the clinical characteristics of COVID-19 cases during the second wave of the pandemic in patients admitted to the only primary care hospital in the district of Rohrbach between October 2020 and February 2021. In total, 260 patients were hospitalized with a mean age of 72 years and a mortality rate of 14.6% and 13 patients (5%) were transferred to the intensive care unit (ICU). ⋯ These laboratory parameters might help to identify COVID-19 patients with a severe disease course. In conclusion, we could show that older, frail individuals are the most vulnerable group affected by COVID-19. Whether this trend in hospitalized patients continues with the persistence of the pandemic, the emergence of novel virus mutations, and the availability of several different vaccines is presently unclear and remains to be determined.

Variation in SARS-CoV-2 molecular test sensitivity by specimen types in a large sample of emergency department patients.
Provider-collected nasopharyngeal specimens for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) molecular testing are the standard of care in many clinical settings, but patient-collected saliva and anterior nares specimens are less invasive and more flexible alternatives. Prior studies comparing specimen types for SARS-CoV-2 molecular testing have been limited by small sample sizes and low pretest probability. We conducted a large observational study among symptomatic adults at 7 emergency departments of Kaiser Permanente Southern California to examine sensitivity of SARS-CoV-2 molecular tests by specimen type and patient characteristics. ⋯ This study provides robust evidence that patient-collected saliva and anterior nares are sensitive for SARS-CoV-2 molecular testing in emergency department settings, particularly among adults ages ≥40 years and those with multiple symptoms. Higher sensitivity of provider-collected NP/OP specimens must be weighed against the benefits of patient-collected specimens in tailored strategies for SARS-CoV-2 testing.

Analyzing Correlation of Clinical Severity of COVID-19 with Other Biochemical Parameters: A Retrospective Study from Pakistan.
The third wave of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is causing damage all over the world, especially in Pakistan and India. Although vaccines are available and preventive measures are being taken, but SARS-CoV-2 is unstoppable. Currently, there are around 841,636 positive cases in Pakistan and 18,429 deaths, whereas, in India, both are high. ⋯ In addition, this study found that COVID-19 patients had significantly higher levels of ferritin, D-dimers, LDH, and CRP. Our study revealed that SARS-CoV-2 relapsed. Furthermore, we concluded that these biochemical parameters are useful indicators for severity of COVID-19.

Rapid point-of-care (POC) SARS-CoV-2 detection with Abbott ID NOW™ COVID-19 test has been implemented in our Emergency Department (ED) for several months. We aimed to evaluate the operational impact and potential benefits of this innovative clinical pathway. ⋯ We demonstrated that COVID-19 point-of-care testing speeds up clinical decision-making, improving use of recommended treatments for COVID-19, such as anticoagulants. Moreover, it improves the boarding time and significantly shortened the length of stay in the ED for patients requiring outpatient care.
