We report a case of acute respiratory distress which occurred three days after orthopaedic surgery and which led to the diagnosis of spontaneous rupture of the oesophagus (Boerhaave syndrome). The unusual nature of the signs delayed diagnosis.

[Surgical treatment of pediatric laryngeal stenosis in the area of the ring cartilage].
In the past 15 years, infants and children with laryngeal stenosis have also been successfully treated with laryngotracheoplasty. From 1991 to 1993 we performed a surgical treatment in 20 pediatric patients with subglottic stenosis. ⋯ In 14 patients with a tracheostoma we used a modification of Réthi's division of the posterior lamina of the cricoid by the placement of a autogenous costal cartilage graft. A 6-week-period of stenting is required for sufficient healing and stability of the reconstructed airway.

Elevated airway pressures during mechanical ventilation are associated with hemodynamic compromise and pulmonary barotrauma. We studied the cardiopulmonary effects of a pressure-limited mode of ventilation (airway pressure release ventilation) in patients with the adult respiratory distress syndrome. ⋯ Our results suggest that while airway pressure release ventilation can provide similar oxygenation and ventilation at lower peak and end-expiratory pressures, this offers no hemodynamic advantages.

This study investigates the course of serum cytokine levels in patients with multiple trauma, patients with a ruptured abdominal aortic aneurysm (AAA), and patients undergoing elective AAA repair and the relationship of these cytokines to the development of adult respiratory distress syndrome (ARDS) and multiple organ failure (MOF). ⋯ In the early postinjury phase, higher concentrations of these cytokines are associated, not only with an increased mortality rate, but also with an increased risk for subsequent ARDS and MOF. These data therefore support the concept that these syndromes are caused by an overwhelming autodestructive inflammatory response.

A novel leukotriene B4-receptor antagonist in endotoxin shock: a prospective, controlled trial in a porcine model.
To evaluate the hypothesis that treatment with LY255283, a novel leukotriene B4-receptor antagonist, is beneficial in an animal model of the adult respiratory distress syndrome induced by endotoxin. ⋯ These data suggest that leukotriene B4 may be an important mediator of acute lung injury in this porcine model of septic shock and acute lung injury. Further studies to assess the specificity of LY255283 as a leukotriene B4 antagonist are necessary in order to exclude the possibility that the beneficial effects of this compound are due to pharmacologic actions other than the blockade of LTB4 receptors.
