AbstractMyopathy as the sole manifestation of hypothyroidism is rare although muscle weakness, aches and cramps, stiffness and delayed tendon jerk relaxation are the usual features of hypothyroid associated myopathy. We describe a patient with primary hypothyroidism, presenting solely with a clinical picture of myositis with very high levels of creatine phosphokinase (CPK), which normalised after 12 weeks of treatment with levothyroxine.
